| This report describe the various human error encountered during Daya bay refueling outage,and the subsequent root cause evaluation and corrective action.Authours consider transparency is the key in reduceing human error and event recurrence.Additionaly,event transparency will enhance the effectiveness if experience feedback and reduce event consequence. |
