| 目的探究胫骨骨膜牵张术对慢性下肢溃疡患者微循环及溃疡愈合的影响。 方法回顾性选择慢性下肢溃疡患者95例,根据治疗方法分为研究组(43例,采用胫骨骨膜牵张术)和对照组(52例,采用常规清创负压引流术)。比较两组患肢术后下肢溃疡愈合率、患肢动脉血运情况、足趾血氧饱和度、足温、疼痛视觉模拟评分法(VAS)评分及并发症情况。 结果研究组下肢溃疡愈合率高于对照组(P<0.05)。与术前比较,术后两组动脉血流量、足趾血氧饱和度及足温均升高,动脉血管阻力、VAS评分均下降,且研究组变化较对照组更为显著(P<0.05)。研究组并发症总发生率低于对照组,但差异无显著性(P>0.05)。 结论胫骨骨膜牵张术治疗慢性下肢溃疡愈合率更高,患肢微循环得到有效改善,且术后并发症发生率也较低。 |

| AimThis study aims to investigate the effects of tibial periosteal distraction on microcirculation and ulcer healing in patients with chronic lower limb ulcers. MethodsA retrospective analysis was conducted on 95 patients with chronic lower limb ulcers. According to the treatment methods, they were divided into a study group (43 cases, treated with tibial periosteal distraction) and a control group (52 cases, treated with conventional debridement and negative pressure drainage). The postoperative lower limb ulcer healing rate, arterial blood supply, toe oxygen saturation, foot temperature, visual analogue scale (VAS) pain score, and complications were compared between the two groups. ResultsThe lower limb ulcer healing rate in the study group was higher than that in the control group (P<0.05). Compared with preoperative values, postoperative arterial blood flow, toe oxygen saturation, and foot temperature increased in both groups, while arterial vascular resistance and VAS scores decreased; these changes were more significant in the study group compared with the control group (P<0.05). The total incidence of complications in the study group was lower than that in the control group, but the difference was not significant (P>0.05). ConclusionTibial periosteal distraction results in a higher healing rate for chronic lower limb ulcers, effectively improves microcirculation in the affected limb, and is associated with a lower postoperative complication rate. |
