| 目的前瞻性探究血清肠凝集素1(ITLN-1)、前蛋白转化酶枯草溶菌素9(PCSK9)、可溶性单核巨噬细胞血红蛋白清道夫受体(sCD163)水平与急性缺血性脑卒中(AIS)病情及预后的关系。 方法选择332例AIS患者(AIS组),按照美国国立卫生院神经功能缺损(NIHSS)评分将其分为轻度组(n=114)、中度组(n=125)和重度组(n=93);根据治疗3个月预后mRS评分分为预后不良组(n=83)和预后良好组(n=249)。选择同期在本院体检健康的332例志愿者为对照组。比较各组血清ITLN-1、PCSK9、sCD163水平和NIHSS评分。预后的影响因素采用Logistic回归分析,各指标对预后的评估价值采用ROC曲线分析和Z检验分析。 结果患者血清ITLN-1水平对照组>AIS组,轻度组>中度组>重度组,预后良好组>预后不良组;PCSK9和sCD163水平对照组<AIS组,轻度组<中度组<重度组,预后良好组<预后不良组;NIHSS评分轻度组<中度组<重度组,预后良好组<预后不良组(P<0.05)。ITLN-1、PCSK9、sCD163均是AIS患者预后的影响因素(P<0.05)。三指标联合对预后评估的效能高于单独指标(P<0.001)。 结论AIS患者血清ITLN-1水平降低,PCSK9、sCD163水平升高,三指标联合检测对AIS患者预后具有良好的评估价值。 |

| AimTo prospectively investigate the relationship between serum levels of intelectin-1 (ITLN-1), proprotein convertase subtilisin/kexin type 9 (PCSK9), and soluble CD163 (sCD163) and disease severity as well as prognosis in acute ischemic stroke (AIS). MethodsA total of 332 AIS patients (AIS group) were enrolled and divided into the mild (n=114), moderate (n=125), and severe (n=93) groups according to the National Institutes of Health Stroke Scale (NIHSS) score. Based on the modified Rankin Scale (mRS) score at 3 months after treatment, patients were classified into the poor prognosis (n=83) and good prognosis (n=249) groups. Additionally, 332 healthy volunteers undergoing physical examinations during the same period were selected as the control group. Serum ITLN-1, PCSK9, and sCD163 levels and NIHSS scores were compared among the groups. Logistic regression analysis was used to identify factors influencing prognosis, and receiver operating characteristic (ROC) curves with Z-test were applied to evaluate the predictive value of each indicator. ResultsSerum ITLN-1 levels were higher in the control group than those in the AIS group, higher in the mild group than those in the moderate and severe groups, and higher in the good prognosis group than those in the poor prognosis group. In contrast, PCSK9 and sCD163 levels were lower in the control group than those in the AIS group, lower in the mild group than those in the moderate and severe groups, and lower in the good prognosis group than those in the poor prognosis group. NIHSS scores were lower in the mild group than those in the moderate and severe groups and lower in the good prognosis group than those in the poor prognosis group (P<0.05). ITLN-1, PCSK9, and sCD163 were all independent factors influencing prognosis in AIS patients (P<0.05). The combined predictive value of the three indicators was higher than that of any single indicator (P<0.001). ConclusionAIS patients exhibit decreased serum ITLN-1 levels and increased PCSK9 and sCD163 levels. The combined detection of these three indicators provides good prognostic evaluation value for AIS patients. |
