      目的探究红外偏振光联合神经肌肉电刺激(NMES)对缺血性脑卒中(IS)患者疗效及血清神经营养因子-3(NT-3)、脑源性神经营养因子(BDNF)及神经生长因子(NGF)的影响。 方法选取IS患者100例,随机均分为对照组(NMES治疗)和研究组(NMES+红外偏振光治疗),每组50例。比较两组神经功能[美国国立卫生研究院卒中量表(NIHSS)评分、中国卒中量表(CSS)评分]、运动功能[上肢动作研究量表(ARAT)、下肢Fugl-Meyer量表(FMA)、肌肉骨骼功能量表(SMFA)、Berg平衡量表(BBS)]、血流动力学指标[动态阻抗(DR)、脑平均血流量(CBF)、血流速度(MBF)]、生活质量[改良Barthel指数(MBI)、卒中专用生活质量量表(SS-QOL)]及血清NT-3、BDNF及NGF水平。 结果与治疗前比较,治疗后两组NIHSS评分、CSS评分、SMFA评分、DR均降低,上肢ARAT评分、下肢FMA评分、BBS评分、CMBI评分、SS-QOL评分、BF、MBF、NT-3、BDNF、NGF水平均升高;且研究组变化较对照组更为显著(P<0.05)。 结论红外偏振光联合NMES治疗IS可明显改善患者神经功能、运动功能及血流动力学指标,提高血清神经营养因子水平,提升患者生活质量。

      AimTo explore the effects of infrared polarized light combined with neuromuscular electrical stimulation (NMES) on the therapeutic efficacy and serum levels of neurotrophin-3 (NT-3), brain-derived neurotrophic factor (BDNF), and nerve growth factor (NGF) in patients with ischemic stroke (IS). MethodsA total of 100 IS patients were selected and randomly and equally divided into a control group (receiving NMES therapy) and a study group (receiving NMES plus infrared polarized light therapy), with 50 patients in each group. Neurological function (national institutes of health stroke scale (NIHSS), China stroke scale (CSS) scores), motor function (action research arm test (ARAT) score for upper limbs, Fugl-Meyer assessment (FMA) score for lower limbs, short musculoskeletal function assessment (SMFA) score, Berg balance scale (BBS) score), hemodynamic parameters (dynamic resistance (DR), blood flow (BF), mean blood flow (MBF), quality of life (modified barthel index (MBI), stroke-specific quality of life scale (SS-QOL) score), and serum levels of NT-3, BDNF, and NGF were compared between the two groups. ResultsCompared with the levels before treatment, after treatment, both groups showed decreased NIHSS scores, CSS scores, SMFA scores, and DR, while upper limb ARAT scores, lower limb FMA scores, BBS scores, MBI scores, SS-QOL scores, BF, MBF, and serum levels of NT-3, BDNF, and NGF increased. These changes were more significant in the study group compared to the control group (P<0.05). ConclusionInfrared polarized light combined with NMES as a way for treating IS can significantly improve patients' neurological function, motor function, and hemodynamic parameters, increase serum neurotrophic factor levels, and enhance patients' quality of life.
