      目的观察苯磺酸瑞马唑仑在肋软骨综合鼻整形术麻醉中的应用。 方法选取行肋软骨综合鼻整形术的90例患者,随机分为瑞马唑仑组(常规+苯磺酸瑞马唑仑)和对照组(常规+丙泊酚)。比较两组麻醉诱导意识消失时间、停药至睁眼时间及气管导管拔除时间;以及麻醉诱导前(T1)、气管插管时(T2)、切皮时(T3)及植入肋软骨时(T4)的平均动脉压(MAP)和心率(HR)。比较两组不良反应发生率。 结果瑞马唑仑组麻醉诱导意识消失时间、停药至睁眼时间、拔管时间均长于对照组(P<0.05)。与T1相比,瑞马唑仑组T2~T4 MAP、HR略降,但差异无显著性(P>0.05)。T2~T4时,对照组MAP、HR略低于瑞马唑仑组(P<0.05),但无临床意义。瑞马唑仑组不良反应发生率明显低于对照组(P<0.05)。 结论苯磺酸瑞马唑仑麻醉诱导起效时间较快,无注射痛,对血流动力学影响小,不良反应少,可安全有效应用于肋软骨综合鼻整形术全麻诱导及维持中。

      AimTo observe the application of remimazolam besylate in anesthesia for comprehensive rib cartilage rhinoplasty. MethodsA total of 90 patients undergoing comprehensive rib cartilage rhinoplasty were randomly divided into a remimazolam group (conventional anesthesia + remimazolam besylate) and a control group (conventional anesthesia + propofol). The time to loss of consciousness during anesthesia induction, time to eye opening after drug discontinuation, and tracheal tube extubation time were compared between the two groups. Mean arterial pressure (MAP) and heart rate (HR) were recorded before anesthesia induction (T1), during tracheal intubation (T2), at skin incision (T3), and during rib cartilage implantation (T4). The incidence of adverse reactions was also compared. ResultsThe time to loss of consciousness, time to eye opening, and extubation time in the remimazolam group were longer than those in the control group (P<0.05). Compared with levels at T1, MAP and HR in the remimazolam group decreased slightly at T2-T4, but the difference was not significant (P>0.05). At T2-T4, MAP and HR in the control group were slightly lower than those in the remimazolam group (P<0.05), but the difference was not clinically significant. The incidence of adverse reactions in the remimazolam group was significantly lower than that in the control group (P<0.05). ConclusionRemimazolam besylate demonstrates a relatively rapid onset of action during anesthesia induction. It has no injection pain, minimal impact on hemodynamics, and a low incidence of adverse reactions. Therefore, it can be safely and effectively applied in the induction and maintenance of general anesthesia for costal cartilage composite rhinoplasty.
