| 目的分析阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者生长刺激表达基因2蛋白(ST2)、血清淀粉样蛋白A(SAA)与颈动脉粥样硬化(As)的关系。 方法回顾性分析147例OSAHS患者临床资料,按颈动脉内膜中膜厚度(CIMT)分为As组(n=67)与非As组(n=80),按Crouse积分将As组分为轻度组(n=24)、中度组(n=23)、重度组(n=20)。比较各组临床资料、ST2、SAA,行二元Logistic回归分析颈As发生及其严重程度的危险因素,行Spearman相关性分析各指标相关性,行ROC曲线分析ST2、SAA对颈As发生及其严重程度的预测价值。 结果As组体质指数、糖尿病和高血压占比、吸烟史、AHI、ST2、SAA均高于非As组(P<0.05)。糖尿病和高血压占比、ST2、SAA轻度组<中度组<重度组(P<0.05)。Spearman相关性分析发现,ST2、SAA均与颈As严重程度呈正相关(P<0.05)。吸烟史、糖尿病、高血压、SAA及ST2是OSAHS合并颈As的危险因素,ST2、SAA是OSAHS合并颈As严重程度的危险因素(P<0.05)。SAA、ST2对OSAHS合并颈As及其严重程度均具有良好的预测效能(P<0.05)。 结论血清ST2、SAA水平可影响OSAHS患者颈As的发生,能有效预测颈As及其严重程度。 |

| AimTo analyze the relationship between growth stimulation expressed gene 2 protein (ST2), serum amyloid A (SAA) levels and carotid atherosclerosis (As) in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). MethodsWe retrospectively analyzed clinical data of 147 OSAHS patients. According to carotid intima-media thickness (CIMT), the patients were divided into an As group (n=67) and a non-As group (n=80). The As group was further divided into mild (n=24), moderate (n=23) and severe (n=20) subgroups based on Crouse scores. We compared general clinical data, ST2 and SAA levels among groups, performed binary Logistic regression to analyze risk factors for carotid As and its severity, conducted Spearman correlation analysis for indicators, and analyzed the predictive value of ST2 and SAA levels for carotid As and its severity using ROC curves. ResultsBody mass index, proportions of diabetes and hypertension, smoking history, AHI, ST2 and SAA levels were higher in As group than those non-As group (P<0.05). These indicators were in an order of mild group < moderate group < severe group (P<0.05). Spearman correlation analysis revealed positive correlations between ST2, SAA levels and carotid As severity (P<0.05). Smoking history, diabetes, hypertension, SAA and ST2 levels were risk factors for OSAHS with carotid As, while ST2 and SAA levels were risk factors for disease severity (P<0.05). SAA and ST2 levels showed good predictive value for OSAHS with carotid As and its severity (P<0.05). ConclusionSerum ST2 and SAA levels may influence carotid As development in OSAHS patients and can effectively predict carotid As and its severity. |
