      目的探讨飞秒激光小切口角膜基质透镜取出术(SMILE)矫正近视的疗效及对视觉质量与眼表状态的影响。 方法选取84例(168眼)行屈光性角膜激光手术患者,根据不同术式分为准分子激光屈光性角膜切削术(PRK)组、飞秒激光辅助准分子激光原位角膜磨镶术(Fs-LASIK)组、飞秒激光小切口角膜基质透镜取出术(SMILE)组。比较3组视力、屈光度、视觉质量、角膜荧光染色、眼表相关综合指标。 结果术后各时间点UCVA、SE组间比较差异无显著性(P>0.05)。术后各组总视觉质量评分均低于术前;PRK组术后各时间点总视觉质量评分低于其他两组(P<0.05)。术后1周及3个月同时间点比较,PRK组角膜荧光染色评分最高,SMILE组最低;术后同时间点比较,PRK组OSDI评分最高,SMILE组最低(P<0.05)。术后各时间点SMILE组TMH、NITBUT、脂质层厚度及NITBUT显著优于Fs-LASIK、PRK组,Fs-LASIK组显著优于PRK组(P<0.05)。 结论PRK、Fs-LASIK与SMILE手术对近视患者视力矫正效果相当,但SMILE手术对眼表指标的影响最小,患者早期视觉质量更高。
英文摘要:
      AimTo investigate the efficacy of small incision lenticule extraction (SMILE) for myopia correction and its impact on visual quality and ocular surface status. MethodsA total of 84 patients (168 eyes) undergoing refractive laser surgery were selected and divided into three groups according to different surgical procedures, namely photorefractive keratectomy (PRK) group, femtosecond laser-assisted in situ keratomileusis (Fs-LASIK) group, and small incision lenticule extraction (SMILE) group. Visual acuity, refractive diopter, visual quality, corneal fluorescein staining, and comprehensive ocular surface-related indicators were compared among the three groups. ResultsNo significant differences were observed in UCVA and SE of the groups at all postoperative time points (P>0.05). The total visual quality scores of all groups postoperatively were lower than preoperative levels, and the PRK group showed lower total visual quality scores than the other two groups at all postoperative time points (P<0.05). At 1 week and 3 months postoperatively, the PRK group had the highest corneal fluorescein staining scores, while the SMILE group had the lowest scores. At all postoperative time points, the PRK group exhibited the highest OSDI scores, and the SMILE group showed the lowest scores (P<0.05). At all postoperative time points, the SMILE group demonstrated significantly better TMH, NITBUT, lipid layer thickness, and NITBUT than the Fs-LASIK and PRK groups, while the Fs-LASIK group exhibited significantly better performance than the PRK group (P<0.05). ConclusionPRK, Fs-LASIK, and SMILE procedures demonstrate comparable efficacy in visual acuity correction for myopic patients. However, SMILE surgery has the least impact on ocular surface indicators and provides superior early postoperative visual quality.
