      目的探讨体外膜肺氧合(ECMO)辅助心肺复苏(CPR)治疗急诊院外心脏骤停(SCA)患者复苏效果的影响因素。 方法回顾性选取268例院外SCA患者,根据复苏情况分为成功组(144例)和失败组(124例)。比较两组临床资料和抢救指标,采用单因素和多因素Logistic回归分析复苏成功的危险因素。 结果失败组SCA绝对时间、急救反应时间、复苏开始时间、CPR-ECMO时间高于成功组,目击者参与CPR、未电除颤占比低于成功组(P<0.05)。SCA绝对时间、复苏开始时间、CPR-ECMO时间和未电除颤均为ECMO辅助CPR复苏成功的独立危险因素(P<0.05)。 结论SCA绝对时间、复苏开始时间、CPR-ECMO时间和未电除颤均为院外SCA患者ECMO辅助CPR复苏成功的危险因素。

      AimTo explore the factors influencing the resuscitation effects of extracorporeal membrane oxygenation (ECMO)-assisted cardiopulmonary resuscitation (CPR) in out-of-hospital sudden cardiac arrest (SCA) patients. MethodsA total of 268 out-of-hospital SCA patients were retrospectively analyzed and divided into a success group (144 cases) and a failure group (124 cases) based on resuscitation outcomes. Clinical data and rescue indicators were compared between the two groups. Univariate and multivariate logistic regression analyses were performed to identify risk factors for successful resuscitation. ResultsThe failure group had longer SCA absolute time, emergency response time, resuscitation start time, and CPR to ECMO time compared to the success group, with lower rates of bystander CPR and defibrillation (P<0.05). SCA absolute time, resuscitation start time, CPR to ECMO time, and absence of defibrillation were identified as independent risk factors for successful resuscitation under ECMO-assisted CPR (P<0.05). ConclusionLonger SCA absolute time, resuscitation start time, CPR to ECMO time, and absence of defibrillation are risk factors for successful resuscitation in out-of-hospital SCA patients under ECMO-assisted CPR.
