目的分析脐带血微小RNA-29b(miR-29b)、磷酸酶张力蛋白同源物(PTEN)表达水平与新生儿低血糖脑损伤的相关性。 方法将本院收治的42例低血糖新生儿,根据头颅MRI结果是否存在脑损伤分为新生儿低血糖症组22例及新生儿低血糖脑损伤组(NHBI组)20例,另选取30例健康足月新生儿为对照组。比较3组脐带血miR-29b、PTEN水平;采用Pearson相关系数分析miR-29b、PTEN与血清胰岛素(INS)、神经元特异性烯醇化酶(NSE)、血糖、新生儿行为神经测定(NBNA)评分的相关性;采用ROC曲线评估脐带血miR-29b、PTEN对新生儿低血糖脑损伤的预测价值。 结果新生儿低血糖症组、NHBI组血清miR-29b相对表达水平均低于对照组(P<0.05);PTEN相对表达水平,NHBI组>新生儿低血糖症组>对照组(P<0.05)。miR-29b与PTEN的3′UTR区域存在结合位点。新生儿脐带血miR-29b与PTEN、INS、NSE呈负相关(P<0.05),与血糖、NBNA评分呈正相关(P<0.05);PTEN与血糖、NBNA评分呈负相关(P<0.05),与INS、NSE呈正相关(P<0.05)。新生儿脐带血miR-29b联合PTEN预测新生儿低血糖脑损伤的AUC值高于其单独预测的AUC值(P<0.05)。 结论脐带血miR-29b、PTEN水平与新生儿低血糖脑损伤的发生、发展有关,且在其诊断和预后评估中具有一定价值。 |

AimTo analyze the correlation between the expression levels of microRNA-29b (miR-29b), phosphatase and tensin homolog (PTEN) in umbilical cord blood and neonatal hypoglycemic brain injury. Methods42 neonates with hypoglycemia admitted to our hospital were divided into a neonatal hypoglycemia group (n=22) and an neonatal hypoglycemic brain injury group (NHBI group, n=20) based on the presence of brain injury on cranial MRI results. Additionally, 30 healthy full-term neonates were selected as the control group. The levels of miR-29b and PTEN in umbilical cord blood of the three groups were compared. Pearson correlation analysis was used to analyze the correlation between miR-29b, PTEN, and serum insulin (INS), neuron-specific enolase (NSE), hypoglycemia, neonatal behavioral neural assessment (NBNA) scores. ROC curve was used to evaluate the predictive value of miR-29b and PTEN in umbilical cord blood for neonatal hypoglycemic brain injury. ResultsThe serum miR-29b relative expression levels in the neonatal hypoglycemia group and the NHBI group were lower than those in the control group (P<0.05). The relative expression level of PTEN was in the order of NHBI group > NHBI group > the control group (P<0.05). There were binding sites in the 3′UTR region of miR-29b and PTEN. miR-29b in neonatal umbilical cord blood was negatively correlated with PTEN, INS, and NSE (P<0.05), while positively correlated with blood glucose ,and NBNA score (P<0.05). PTEN was negatively correlated with blood glucose and NBNA score (P<0.05), but positively correlated with blood glucose, INS, and NSE (P<0.05). The AUC of miR-29b combined with PTEN in predicting neonatal hypoglycemic brain injury in umbilical cord blood was higher than its individual prediction value (P<0.05). ConclusionThe levels of miR-29b and PTEN in umbilical cord blood are related to the occurrence and development of neonatal hypoglycemic brain injury, and have certain value in the diagnosis and prognosis evaluation of neonatal hypoglycemic brain injury, which can provide reference for clinical treatment. |
