| 目的分析Angiojet机械血栓清除联合导管接触溶栓(CDT)治疗下肢深静脉血栓(DVT)的疗效。 方法选取采用CDT治疗的DVT患者137例,其中69例患者联合Angiojet机械血栓清除手术(联合组),其余患者纳入溶栓组(n＝68)。比较两组腿周径差值、静脉畅通评分、凝血指标[纤维蛋白(FIB)、D-二聚体(D-D)、血小板计数(PLT)]、血管细胞黏附分子-1(VCAM-1)、血小板内皮细胞黏附分子-1(PECAM-1)、药物使用、住院时间以及并发症等情况。 结果与术前比较,术后两组患者的健侧小腿周径差值、大腿周径差值、静脉畅通评分均减小；且术后联合组均小于溶栓组(P＜0.05)。与术前比较,两组术后凝血指标和炎症反应指标均减小；且术后联合组FIB高于溶栓组,VCAM-1、PECAM-1低于溶栓组(P＜0.05)。联合组尿激酶用量小于溶栓组,溶栓时间、住院时间短于溶栓组(P＜0.05)。联合组并发症发生率低于溶栓组(P＜0.05)。 结论DVT患者采用Angiojet机械血栓清除联合CDT治疗方案,能更早溶栓和改善血管闭塞病情。 |

| AimTo analyze the efficacy of Angiojet mechanical thrombectomy combined with catheter directed thrombolysis (CDT) in the treatment of lower extremity deep vein thrombosis (DVT). Methods137 patients with DVT treated with catheter directed thrombolysis were selected, including 69 patients in combination with Angiojet mechanical thrombectomy (combination group) and the rest in the thrombolysis group (n＝68). The difference in leg circumference, venous patency score, coagulation indicators (fibrinogen (FIB), D-dimer (D-D), platelet count (PLT), vascular cell adhesion molecule-1 (VCAM-1), platelet endothelial cell adhesion molecule-1 (PECAM-1), drug use, length of hospital stay, and complications between two groups were compared. ResultsCompared with preoperative results, the difference in circumference of the healthy thigh and calf, as well as the venous patency score, decreased in both groups after surgery; And the postoperative combined group was smaller than the thrombolysis group (P＜0.05). Compared with preoperative results, both groups showed a decrease in postoperative coagulation and inflammatory response indicators; Moreover, the postoperative fibrinogen level in the combined group was higher than that in the thrombolysis group, while VCAM-1 and PECAM-1 levels were lower than those in the thrombolysis group (P＜0.05). The dosage of urokinase in the combination group was lower than that in the thrombolysis group, and the thrombolysis time and hospitalization time were shorter than those in the thrombolysis group (P＜0.05). The incidence of complications in the combination group was lower than that in the thrombolysis group (P＜0.05). ConclusionThe Angiojet mechanical thrombectomy combined with catheter directed thrombolysis treatment for DVT patients can achieve earlier thrombolysis and improve vascular occlusion. |
