      目的分析脐血维生素A(VA)水平与新生儿牛奶蛋白过敏(CMPA)的相关性。 方法选取本院分娩的92例CMPA新生儿为CMPA组,同期健康新生儿92例为对照组。CMPA组患儿根据临床表现分为轻中度组67例和重度组25例。比较各组新生儿脐血及新生儿血VA、新生儿血嗜酸性粒细胞数目(EC)、免疫球蛋白E(IgE)和淋巴细胞亚群水平。采用相关性分析脐血VA与新生儿血VA、EC、IgE及淋巴细胞亚群的相关性;采用ROC曲线分析脐血VA对新生儿发生CMPA的预测价值。 结果脐血VA、新生儿血VA、CD3+、CD3+CD4+、CD3+CD8+水平为对照组>轻中度组>重度组,而新生儿血IgE、EC、CD3-CD19+为对照组<轻中度组<重度组。在轻中度组、重度组及184例新生儿中,脐血VA与新生儿血CD3+、CD3+CD4+、CD3+CD8+均呈正相关(P<0.05),与IgE、EC、CD3-CD19+均呈负相关(P<0.05);在重度组及184例新生儿中,脐血VA与新生儿血VA呈正相关(P<0.05)。脐血VA对CMPA的发生及CMPA的严重程度均具有一定的预测价值(P<0.05)。 结论VA缺乏可能会导致新生儿出现CMPA,且与CMPA严重程度具有相关性。

      AimTo explore the correlation between umbilical blood vitamin A (VA) level and neonatal cow milk protein allergy (CMPA). Methods92 CMPA newborns in our hospital were selected as the CMPA group, and 92 healthy newborns were selected as the control group. According to clinical manifestations, the children in the CMPA group were divided into mild to moderate group (67 cases) and severe group (25 cases). The levels of VA in umbilical blood and neonatal blood, and eosinophils count (EC), immunoglobulin E (IgE) and lymphocyte subsets in neonatal blood among different groups were compared. The correlation between umbilical blood VA and neonatal blood VA, EC, IgE and lymphocyte subsets was analyzed. ROC curve was used to analyze the predictive value of umbilical blood VA for neonatal CMPA. ResultsThe levels of umbilical blood VA, neonatal blood VA, CD3+, CD3+CD4+, CD3+CD8+ follow an order of control group > mild to moderate group > severe group, while neonatal blood IgE, EC, CD3-CD19+ were in an order of control group< mild to moderate group< severe group. In the mild to moderate group, severe group and 184 newborns, umbilical blood VA was positively correlated with neonatal blood CD3+, CD3+CD4+, CD3+CD8+ (P<0.05), while negatively correlated with IgE, EC, CD3-CD19+ (P<0.05); In the severe group and 184 newborns, there was a positive correlation between umbilical blood VA and neonatal blood VA (P<0 05). Cord blood VA had a certain predictive value for the occurrence and severity of CMPA (P<0.05). ConclusionVA deficiency may lead to CMPA in newborns and is associated with CMPA severity.
