      目的分析血清缺氧诱导因子-1α(HIF-1α)及成纤维细胞生长因子21(FGF21)水平对急性心肌梗死(AMI)患者心室重构的预测价值。 方法选取本院AMI患者202例为研究对象,并根据有无心室重构将患者分为未重构组105例、早期重构组46例及晚期重构组51例。比较各组患者一般资料、入院时血清HIF-1α、FGF21水平。采用多因素Logistic回归分析心肌梗死患者心室重构的影响因素;ROC曲线分析血清HIF-1α、FGF21预测心肌梗死后心室重构的价值。 结果与未重构组比较,早期重构组体质指数(BMI)、前壁梗死、病变数≥2支、慢性完全闭塞病变、Killip≥3级占比及HIF-1α水平升高,入院左室射血分数(LVEF)、FGF21水平降低(P<0.05);晚期重构组BMI、病变数≥2支占比升高,入院LVEF、HIF-1α、FGF21水平降低(P<0.05)。Logictic回归分析结果显示,前壁梗死、高BMI、HIF-1α水平及低FGF21水平是早期心室重构的危险因素;高BMI、低LVEF、HIF-1α及FGF21水平是晚期心室重构的危险因素。血清HIF-1α、FGF21对早期、晚期心室重构的预测均有一定的价值,且两者联合预测效能高于其单独预测效能。 结论血清HIF-1α、FGF21水平对心室重构具有一定的预测价值,且两者联合预测效能高于其单独预测效能。

      AimTo analyze the predictive value of blood hypoxia-inducible factor-1α (HIF-1α) and fibroblast growth factor 21 (FGF21) levels for ventricular remodeling in acute myocardial infarction (AMI). MethodsA total of 202 patients with AMI from our hospital were selected as study subjects. The patients were divided into the non-remodeling group (105 cases), early remodeling group (46 cases), and late remodeling group (51 cases) based on the presence or absence of ventricular remodeling. The general data, serum HIF-1α, and FGF21 levels upon admission were compared among the groups. Multivariate Logistic regression analysis was performed to identify the factors influencing ventricular remodeling in AMI patients. ROC curve analysis was conducted to evaluate the predictive value of serum HIF-1α and FGF21 for post-myocardial infarction ventricular remodeling. ResultsCompared with the non-reconstructed group, the early remodeling group showed an increase in body mass index (BMI), proportion of anterior wall infarction, number of lesions ≥2, chronic total occlusion, Killip ≥3 grade, and HIF-1α level, while left ventricular ejection fraction (LVEF) and FGF21 level were decreased (P<0.05); The late remodeling group showed an increase in BMI and the proportion of number of lesions ≥2, while a decrease in LVEF, HIF-1α, and FGF21 levels (P<0.05). Logistic regression analysis indicated that anterior wall infarction, high BMI, HIF-1α level, and low FGF21 level were risk factors for early ventricular remodeling. High BMI, low LVEF, HIF-1α and FGF21 level were identified as risk factors for late ventricular remodeling. Serum HIF-1α and FGF21 levels have predictive value for both early and late ventricular remodeling, and their combined detection demonstrates higher predictive performance than individual tests. ConclusionSerum HIF-1α and FGF21 levels have predictive value for both early and late ventricular remodeling, and their combined detection demonstrates higher predictive performance than individual tests.
