      目的探讨支气管哮喘患者血清降钙素基因相关肽(CGRP)和趋化因子水平与气道炎症及疾病进展的关系。 方法将支气管哮喘患者61例分为急性组(急性发作期患者37例)和慢性组(急性发作期患者37例)。比较两组患者一般资料和实验室检测结果,通过Logistic模型分析支气管哮喘急性发作的危险因素。 结果急性组年龄、体质指数、血清CGRP和趋化因子水平、诱导痰炎症细胞类型指标和炎症相关细胞因子水平均高于慢性组(P<0.05)。Logistic分析结果显示,血清CGRP、CXCL13、MIP-1α、MCP-1水平、诱导痰IL-4、IL-9、嗜酸性粒细胞百分比均为支气管哮喘急性发作的危险因素(P<0.05)。ROC曲线分析结果显示,血清CGRP和趋化因子水平联合检测的支气管哮喘急性发作判断效能最佳。Pearson相关分析结果显示,支气管哮喘患者血清CGRP和MIP-1α水平均与诱导痰炎症细胞类型指标和炎症相关细胞因子水平呈正相关(P<0.05);血清CXCL13与诱导痰IL-6、IL-9、嗜酸性粒细胞百分比、中性粒细胞百分比均呈正相关(P<0.05);血清MCP-1水平与诱导痰IL-6、IL-13、中性粒细胞百分比均呈正相关(P<0.05)。 结论支气管哮喘患者血清CGRP和趋化因子水平与其气道炎症及疾病进展均密切相关。

      AimTo investigate the association of serum calcitonin gene related peptide (CGRP) and chemokines levels with airway inflammation and disease progression in patients with bronchial asthma. Methods61 patients diagnosed with bronchial asthma were selected as the acute group (37 patients with acute exacerbation) and the chronic group (24 patients with chronic duration). The general information and laboratory test results of two groups of patients were compared, and the risk factors for acute exacerbation of bronchial asthma were analyzed by Logistic model. ResultsThe age, body mass index, serum CGRP and chemokine levels, induced sputum inflammatory cell type indicators, and inflammatory related cytokine levels in the acute group were higher than those in the chronic group (P<0.05). The results of Logistic analysis showed that serum CGRP, CXCL13, MIP-1 α, MCP-1 levels, induced sputum IL-4, IL-9, and percentage of eosinophils, were all risk factors for acute exacerbation of bronchial asthma (P<0.05).The ROC curve analysis results showed that combined detection of serum CGRP and chemokine levels had the best diagnostic efficacy for acute exacerbation of bronchial asthma. The Pearson correlation analysis results showed that the serum CGRP and MIP-1α level in patients with bronchial asthma was positively correlated with induced sputum inflammatory cell type indicators and inflammation-related cytokine levels (P<0.05); Serum CXCL13 was positively correlated with induced sputum IL-6, IL-9, eosinophil percentage and neutrophil percentage (P<0.05); Serum MCP-1 level was positively correlated with induced sputum IL-6, IL-13 and neutrophil percentage (P<0.05). ConclusionSerum CGRP and chemokines levels in patients with bronchial asthma are closely related to airway inflammation and disease progression.
