目的分析不同供区指动脉背侧支血管链皮瓣修复对指端缺损患者手指功能的影响。 方法回顾性分析本院接受指动脉背侧支血管链皮瓣修复治疗的指端缺损患者80例,根据皮瓣的不同供区分为近节背侧组40例和中节背侧组40例。比较两组患者围术期指标(手术时间、愈合时间以及两点分辨觉)、术后恢复情况(外观满意度、痛觉敏感、感觉麻木及瘢痕形成)、手术前后关节活动度和手指外观及功能情况。 结果中节背侧组愈合时间、两点分辨觉、瘢痕形成发生率均低于近节背侧组(P<0.05),外观满意度高于近节背侧组(P<0.05)。术后两组关节活动度、手指外观及功能评分均升高(P<0.05),且中节背侧组高于近节背侧组(P<0.05)。两组手术时间、痛觉敏感、感觉麻木发生率比较,差异无显著性(P>0.05)。 结论指动脉背侧支血管链皮瓣修复治疗指端缺损疗效确切,不同供区疗效不同;相较于近节背侧支,中节背侧支为供区的血管链皮瓣修复能更有效地促进患指恢复,提升患指关节活动度及手指功能。 |

AimTo analyze the effects of digital artery dorsal branch vascular chain flap repair with different donor sites on finger function in patients with fingertip defect. MethodsA retrospective analysis of 80 patients with fingertip defects treated with digital artery dorsal branch vascular chain flap repair in our hospital was performed. According to the different supply of the flap, 40 cases were divided into the proximal dorsal group and 40 cases in the middle dorsal group. The perioperative indexes (operation time, healing time and two-point discrimination), postoperative recovery (appearance satisfaction, pain sensitivity, sensory numbness and scar formation), joint mobility and finger appearance and function before and after operation were compared between the two groups. ResultsThe healing time, two-point discrimination and the incidence of scar formation in the middle dorsal group were lower than those in the proximal dorsal group (P<0.05). The appearance satisfaction was higher than that in the proximal dorsal group (P<0.05). After operation, the joint mobility, finger appearance and function scores of the two groups were increased (P<0.05), and the middle dorsal group was higher than the proximal dorsal group (P<0.05). There was no significant difference in the operation time, the incidence of pain sensitivity and sensory numbness between the two groups (P>0.05). ConclusionVascular chain flap repair of the digital artery dorsal branch is effective in the treatment of fingertip defect, and the curative effect is different in different donor sites. Compared with the dorsal side of the proximal segment, the vascular chain flap repair with the dorsal side of the middle segment as the donor area can more effectively promote the recovery of the affected finger and improve the joint mobility and finger function of the affected fingers. |
