      目的探讨丹参酮ⅡA对2型糖尿病合并冠心病患者的临床疗效及TLR4/NF-κB通路的影响。 方法选取2型糖尿病合并冠心病患者71例,分成对照组和丹参酮ⅡA组。比较两组血糖、血清超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-10、TLR4、NF-κB mRNA水平及血管舒张功能指标。Pearson分析TLR4/NF-κB通路相关因子水平与血管舒张功能的相关性。 结果与对照组相比,丹参酮ⅡA组糖化血红蛋白、空腹血糖、餐后2 h血糖、血清hs-CRP、TNF-α、IL-6、TLR4 mRNA、NF-κB mRNA水平下降,IL-10水平上升,肱动脉血管内皮依赖的舒张功能(FMD)和踝肱指数(ABI)明显改善(P<0.05)。Pearson相关性分析结果显示,血清hs-CRP、TNF-α、IL-6、TLR4 mRNA、NF-κB mRNA水平与FMD、ABI呈负相关(P<0.05),IL-10与FMD及ABI呈正相关(P<0.05)。 结论丹参酮ⅡA治疗2型糖尿病合并冠心病患者疗效显著,可能与其抑制TLR4/NF-κB信号通路及炎症因子水平相关。

      AimTo explore the clinical effect of tanshinone ⅡA on type 2 diabetes patients with coronary heart disease and the influence of TLR4/NF-κB pathway. Methods71 patients with type 2 diabetes mellitus with coronary heart disease were selected and randomly divided into the control group and the tanshinone ⅡA group, respectively. The blood glucose content and the levels of TLR4/NF-κB signaling pathway-related factors, including high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α), interleukin (IL)-6, IL-10 and serum TLR4 mRNA, NF-κB mRNA and vasodilation indicators, were compared between the two groups. The correlation between the levels of TLR4/NF-κB signaling pathway-related factors and vasodilation was analyzed. ResultsCompared with the control group, the levels of glycosylated hemoglobin, fasting blood glucose, 2 h postprandial blood glucose, hs-CRP, TNF-α, IL-6, TLR4 mRNA and NF-κB mRNA in the tanshinone ⅡA group were significantly decreased, the level of IL-10 was increased, and the brachial artery endothelium-dependent relaxation function (FMD) and ankle brachial index (ABI) was significantly improved (P<0.05). Pearson correlation analysis showed that serum hs-CRP, TNF-α, IL-6, TLR4 mRNA, NF-κB mRNA levels were negatively correlated with FMD and ABI, whereas IL-10 was positively correlated with FMD and ABI (P<0.05). ConclusionTanshinone ⅡA can improve vasodilation in patients with type 2 diabetes and coronary heart disease, which may be related to the inhibition of TLR4/NF-κB signaling pathway and the level of inflammatory factors.
