目的探讨自发性脑出血(SICH)患者微创血肿清除术后血尿酸、中性粒细胞/淋巴细胞比值(NLR)水平与神经功能缺损及预后的相关性。 方法选取行微创血肿清除术SICH患者108例,根据出院后6个月预后情况分为预后良好组(n=74)和预后不良组(n=34)。对比两组临床资料、血尿酸、NLR、神经功能缺损程度[美国国立卫生研究院卒中量表(NIHSS)],采用Pearson相关分析血尿酸、NLR水平与NIHSS评分的相关性,二元Logistic回归分析预后影响因素,ROC曲线分析各指标对预后不良预测价值。 结果预后良好组血尿酸、NLR水平、NIHSS评分均低于预后不良组(P<0.05)。血尿酸、NLR水平与NIHSS评分呈正相关(P<0.05)。年龄>65岁、出血量多、高血尿酸、高NLR、高NIHSS评分为预后不良的独立危险因素(P<0.05)。ROC曲线分析发现,血尿酸、NLR水平对自发性脑出血患者经微创血肿清除术后患者预后不良具有一定预测价值。 结论SICH患者血尿酸和NLR水平与神经功能缺损及预后显著相关;其联合预测对预后具有一定价值。 |

AimTo investigate the correlation of serum uric acid and neutrophil to lymphocyte ratio (NLR) levels with neurological impairment and prognosis in patients with spontaneous intracerebral hemorrhage (SICH) after minimally invasive hematoma removal. Methods108 SICH patients were prospectively selected and divided into good prognosis group (n=74) and poor prognosis group (n=34) according to their prognosis 6 months after discharge. The clinical data, blood uric acid, NLR level, and degree of neurological impairment of the two groups were compared. The correlation between blood uric acid, NLR level and national Institute of Health stroke scale (NIHSS) was analyzed by Pearson correlation, the influencing factors of prognosis were analyzed by binary Logistic regression, and the predictive value of poor prognosis was analyzed by ROC curve. ResultsThe levels of uric acid, NLR and NIHSS in the group with good prognosis were lower than those in the poor prognosis group (P<0.05). Serum uric acid and NLR levels were positively correlated with NIHSS score (P<0.05). Age >65 years old, excessive bleeding, high blood uric acid, NLR, NIHSS scores were independent risk factors for poor prognosis (P<0.05). ROC curve analysis found that blood uric acid and NLR levels have certain predictive value for poor prognosis in patients with spontaneous cerebral hemorrhage after minimally invasive hematoma clearance surgery. ConclusionSerum uric acid and NLR levels in SICH patients are significantly correlated with neurological impairment and prognosis. Combined prediction is valuable for prognosis. |
