目的分析甲型H1N1流感重症肺炎患儿病原学特点及其外周血单个核细胞Nod样受体蛋白3(NLRP3)炎症小体表达。 方法选取196例甲型H1N1流感肺炎患儿,根据肺炎严重程度分为重症肺炎组(n=98)和普通肺炎组(n=98),并选取同期98例体检正常儿童为健康组。观察两组肺炎患儿病原菌分布情况,比较3组NLRP3 mRNA、白细胞介素(IL)-1β、IL-18、超敏C反应蛋白(hs-CRP)水平。分析NLRP3 mRNA与IL-1β、IL-18、hs-CRP水平的相关性,比较不同NLRP3 mRNA水平重症肺炎患儿的儿童早期预警评分(PEWS)。 结果与普通肺炎组比较,重症肺炎组肺炎链球菌检出率升高,流感嗜血杆菌检出率降低(P<0.05)。与健康组比较,普通肺炎组和重症肺炎组NLRP3 mRNA、IL-1β、IL-18、hs-CRP水平均升高,且重症肺炎组较普通肺炎组更为明显(P<0.05)。重症肺炎患儿NLRP3 mRNA水平与IL-1β、IL-18、hs-CRP水平均呈正相关(P<0.05)。治疗后7天,NLRP3 mRNA高表达组PEWS评分高于低表达组(P<0.05)。 结论甲型H1N1流感感染重症肺炎患儿外周血单个核细胞NLRP3 mRNA表达水平增加,可能与预后风险有关。 |

AimTo analyze etiological characteristics and expression of NOD-like receptor protein 3 (NLRP3) inflammasome in peripheral blood mononuclear cells in children with severe pneumonia secondary to influenza A (H1N1). Methods196 children with H1N1 pneumonia were enrolled. According to pneumonia severity, they were divided into severe pneumonia group (n=98) and common pneumonia group (n=98). 98 healthy children during the same period were enrolled as healthy group. The distribution of pathogens in the two pneumonia groups was observed. The levels of NLRP3 mRNA, interleukin (IL)-1β, IL-18 and hypersensitive C reactive protein (hs-CRP) in the three groups were compared. The correlation between NLRP3 mRNA and IL-1β, IL-18, hs-CRP was analyzed. Pediatric early warning score (PEWS) among severe pneumonia children with different level of NLRP3 mRNA was compared. ResultsCompared with the common pneumonia group, the detection rate of Streptococcus pneumoniae increased and the detection rate of Haemophilus influenzae decreased in the severe pneumonia group (P<0.05). Compared with the healthy group, the levels of NLRP3 mRNA, IL-1 β, IL-18, and hs CRP were increased in both the ordinary pneumonia group and the severe pneumonia group, and these parameters were more significant in the severe pneumonia group compared with the ordinary pneumonia group (P<0.05).The level of NLRP3 mRNA in children with severe pneumonia was positively correlated with the levels of IL-1β, IL-18, and hs CRP (P<0.05). After 7 days of treatment, the PEWS score of the NLRP3 mRNA high expression group was higher than that of the low expression group (P<0.05). ConclusionThe expression level of NLRP3 mRNA in peripheral blood mononuclear cells is increased in children with severe pneumonia secondary to influenza A (H1N1), which may be related to prognosis risk. |
