目的观察孕早期子宫动脉血流参数联合血清可溶性血管内皮生长因子受体-1(sFlt-1)、胎盘生长因子(PGF)对妊娠期高血压的预测价值。 方法选取98例妊娠期高血压疾病患者为观察组,根据病情分为轻度组、中度组、重度组;另选取同期80例健康孕妇作为对照组。比较各组孕早期子宫动脉血流参数及血清sFlt-1、PGF水平;采用ROC曲线分析各指标对妊娠期高血压的预测价值。 结果观察组孕早期收缩期最大血流速度/舒张期最大血流速度(S/D)、阻力指数(RI)及搏动指数(PI)及sFlt-1水平较对照组高,且中重度组>中度组>轻度组(P<0.05);观察组PGF水平较对照组低,且中重度组<中度组<轻度组(P<0.05)。Spearman相关性分析显示,妊娠期高血压病情严重程度与孕早期S/D、RI、PI、血清sFlt-1水平呈正相关,与PGF水平呈负相关(P<0.05)。ROC曲线分析显示,孕早期S/D、RI、PI、血清sFlt-1、PGF水平对妊娠期高血压均具有预测价值,且联合检测预测效能相对更高(P<0.05)。 结论孕早期子宫动脉血流参数、血清sFlt-1、PGF水平对妊娠期高血压具有预测价值,且联合检测有助于提高预测效能。 |

AimTo observe the predictive value of early pregnancy uterine artery blood flow parameters combined with serum soluble vascular endothelial growth factor receptor-1 (sFlt-1) and placental growth factor (PGF) for gestational hypertension. Methods98 patients with gestational hypertension were selected as the observation group, and according to the condition, they were divided into mild group, moderate group, and severe group; Another 80 healthy pregnant women from the same period were selected as the control group. The uterine artery blood flow parameters and serum levels of sFlt-1 and PGF in different groups during early pregnancy were compared. ROC curve analysis was used to assess its predictive value for gestational hypertension. ResultsThe levels of peak systolic velocity/peak diastolic velocity (S/D), resistance index (RI), pulsitility index (PI) and sFlt-1 in the observation group during early pregnancy were higher than those in the control group, in an order of moderate to severe group>moderate group>mild group (P<0.05); The PGF level in the observation group was lower than that in the control group, and the moderate to severe group was less than the moderate group and less than the mild group (P<0.05). Spearman correlation analysis showed that the severity of gestational hypertension was positively correlated with early pregnancy S/D, RI, PI, and serum sFlt-1 levels, and negatively correlated with PGF levels (P<0.05). ROC curve analysis showed that the levels of S/D, RI, PI, serum sFlt-1, and PGF in early pregnancy have predictive value for gestational hypertension, and the combined detection has a relatively higher predictive power (P<0.05). ConclusionEarly pregnancy uterine artery blood flow parameters, serum sFlt-1, and PGF levels have predictive value for gestational hypertension, and combined detection can help improve predictive efficiency. |
