目的探讨iRoot SP在微创根管治疗中的充填效果,并探讨其对患者血清CX3C趋化因子配体1(CX3CL1)/Toll样受体4(TLR4)的影响。 方法选取86例需进行磨牙微创根管治疗的患者,随机均分为观察组和对照组。观察组采用iRoot SP进行根管治疗,对照组采用微创根管治疗。比较两组患者血清CX3CL1、TLR4、缺氧诱导因子-1α(HIF-1α)和白细胞分化抗原-14(CD14)水平。对所有患者进行半年随访,观察两组患者的治疗效果,并通过根管充填效果评估标准进行评价;记录牙周探诊以及并发症。 结果观察组术后1、7天根管填充疗效高于对照组(P<0.05)。术后7天,观察组CX3CL1、TLR4、HIF-1α和CD14含量均低于对照组(P<0.05)。术后6个月,观察组牙周袋深度、菌斑指数和龈沟出血指数均低于对照组(P<0.05)。两组患者随访期间,牙龈萎缩、口腔感染、咬合痛、龋齿的发生率差异均无显著性(P>0.05),但观察组并发症总发生率低于对照组(P<0.05)。 结论iRoot SP在微创根管治疗中可提高根管治疗的成功率,还可降低血清CX3CL1、TLR4水平,调节免疫反应以降低感染风险。 |

AimTo investigate the root canal filling effect of iRoot SP in minimally invasive root canal therapy, and to explore the role of serum CX3C chemokine ligand 1 (CX3CL1)/Toll-like receptor 4 (TLR4). Methods86 patients requiring root canal treatment for molars were randomly divided into an observation group and a control group. The observation group received root canal treatment by using iRoot SP material, while the control group received traditional root canal treatment.Through a 6-month follow-up after surgery, the treatment effects of the two groups of patients were observed and evaluated by using the evaluation criteria for root canal filling effectiveness. Serum samples from two groups of patients were collected, and the levels of CX3CL1, TLR4, hypoxia inducible factor-1 alpha (HIF-1 alpha), and leukocyte differentiation antigen-14 (CD14) were measured. A six-month follow-up was coducted for all patients, including periodontal exploration, and recording of complications. ResultsThe therapeutic effect of root canal filling in the observation group was higher than that in the control group 7 days after surgery (P<0.05). Seven days after surgery, the levels of CX3CL1, TLR4, HIF-1 α, and CD14 in the observation group were lower than those in the control group (P<0.05). Six months after surgery, the periodontal pocket depth, plaque index, and gingival bleeding index in the observation group were all lower than those in the control group (P<0.05). During the follow-up period, there was no significant difference in the incidence of gingival recession, oral infection, bite pain, and dental caries between the two groups of patients (P>0.05). The total incidence of complications in the observation group was lower than that in the control group (P<0.05). ConclusioniRoot SP can improve the success rate of minimally invasive root canal therapy, reduce serum levels of CX3CL1 and TLR4, and regulate immune response to reduce infection risk. |
