目的探讨胰激肽原酶(PK)与吡拉西坦片(Piracetam)联合治疗老年2型糖尿病认知功能障碍(T2DM-CI)的疗效及其对细胞色素C(Cyt-C)/Caspase信号通路的调控作用。 方法共86名T2DM-CI患者被随机均分为观察组和对照组。对照组采用Piracetam治疗,观察组在对照组基础上增加PK治疗。比较两组血糖、血脂、神经功能评分、肾功能指标、认知功能评分、日常生活能力评分以及Cyt-C和Caspase-3水平。 结果两组患者治疗后的血糖指标、总胆固醇、甘油三酯、低密度脂蛋白胆固醇水平和Cyt-C、Caspase-3水平均低于治疗前,且观察组低于对照组(P<0.05);治疗后的高密度脂蛋白胆固醇、简易精神状态评价量表评分和日常生活活动评分(ADL)评分均高于治疗前,且观察组高于对照组(P<0.05)。两组治疗后的胰岛素敏感性指数和胰岛素抵抗的同稳态评估模型均较治疗前改善,且观察组较对照组更明显(P<0.05)。 结论PK和Piracetam联合治疗能有效改善老年T2DM-CI患者的血糖控制和认知功能,可能与调节Cyt-C/Caspase信号通路有关。 |

AimTo explore the efficacy of combined treatment with Kallikrein and Piracetam in elderly patients with type 2 diabetes mellitus cognitive impairment (T2DM-CI) and its regulatory effects on the Cytochrome C (Cyt-C)/Caspase signaling pathway. Methods86 T2DM-CI patients were randomly divided into an observation group and a control group. The control group received 12 weeks of treatment, whereas t he observation group received Kallikrein treatment in addition to conventional hypoglycemic therapy and Piracetam. The study compared changes in blood sugar, blood lipids, neurological function scores, renal function indicators, cognitive function scores, daily living ability scores, and levels of Cyt-C and Caspase-3 between the two groups. ResultsAfter treatment, levels of fasting plasma glucose (FPG), total cholesterol (TC), Cyt-C, and Caspase-3 decreased in both groups, with more significant reductions in the observation group (P<0.05). Post-treatment scores for high-density lipoprotein cholesterol (HDLC), MMSE, and ADL were higher than those before treatment, especially in the observation group (P<0.05). Improvements in ISI and HOMA-IR were also more notable in the observation group after treatment (P<0.05). ConclusionCombined treatment with Kallikrein and Piracetam effectively improves blood sugar control and cognitive function in elderly patients with T2DM-CI, possibly related to the modulation of the Cyt-C/Caspase signaling pathway. |
