      目的探究聚合酶链反应-序列特异性引物(PCR-SSP)技术联合抗人球蛋白卡式法在ABO和RhD血型鉴定困难患者中的应用。 方法选取118例ABO与RhD血型鉴定困难患者血型标本,分别应用PCR-SSP技术、抗人球蛋白卡式法分析患者血型情况。 结果118例血型鉴定困难者采用抗人球蛋白卡式法联合PCR-SSP技术明确检出ABO血型113例,同时应用PCR-SSP可以确定ABO亚型,5例未检出血型的样品经PCR-SSP测序发现,基因分型分别为BO02、A205O01、A102B01、A201B01、B01B01,分别出现第7外显子640A>G杂合突变、第7外显子1009A>G杂合突变、第3外显子150G>C杂合突变、第6外显子240T>C、第7外显子431G>T杂合突变;采用抗人球蛋白卡式法联合PCR-SSP检出RhD血型114例,未检出的4例进行测序,1例显示为RhD weak D 779A/G突变;3例为RhD weak D type 4.0(包括602C/G、667T/G以及819G/A突变)。 结论ABO和RhD血型鉴定困难患者应用PCR-SSP技术联合抗人球蛋白卡式法进行鉴定可以准确鉴定血型,并发现血型具体亚型,保证输血安全。

      AimTo explore the application of polymerase reaction sequence specific primers (PCR-SSP) technology combined with anti human globulin cartridge assay in patients with difficulty in ABO and RhD blood type identification. MethodsBlood samples of 118 patients with difficulty in ABO/RhD blood type identification were collected for blood typing by PCR-SSP and anti-human globulin cassette method. ResultsFor the 118 patients, anti-human globulin cassette method and PCR-SSP confirmed that there were 113 patients with ABO type. The ABO subtypes were determined by PCR-SSP. For 5 undetermined samples, PCR-SSP sequencing found the genotypes were BO02, A205O01, A102B01, A201B01 and B01B01, which exhibited the 7th exon 640A>G heterozygous mutation, the 7th exon 1009A>G heterozygous mutation, the 3rd exon 150G>C heterozygous mutation, the 6 exon 240T>C, and the 7th exon 431G>T heterozygous mutation. Anti-human globulin cassette method combined with PCR-SSP detected 114 cases of RhD type, and 4 cases not detected were subjected to sequencing. 1 case showed RhD weak D 779A/G mutation, and 3 cases showed RhD weak D type 4.0 (including 602C/G, 667T/G and 819G/A mutations). ConclusionThe application of PCR-SSP combined with anti-human globulin cassette method in patients with difficulty in ABO/RhD blood type identification can help identify blood types accurately, and find out specific subtypes of blood types, thus improving the safety of blood transfusion.
