| 目的分析血小板/淋巴细胞(PLR)、高敏心肌肌钙蛋白T(hs-cTnT)、血糖(GLU)联合检测对急性胰腺炎(AP)患者预后的预测价值。 方法回顾性分析157例AP患者资料,根据预后情况将患者分为预后良好组135例和预后不良组22例。比较两组一般资料、PLR、血清hs-cTnT及GLU水平；采用Pearson相关系数分析PLR、hs-cTnT、GLU水平与CT严重度指数(CTSI)评分的相关性；采用Logistic回归分析影响AP患者预后的影响因素；采用ROC曲线分析PLR、hs-cTnT、GLU联合检测对AP患者预后的评估价值。 结果预后不良组重症AP患者占比、CTSI≥3分占比、体温、白细胞计数、血清淀粉酶(AMY)、血肌酐、血尿素氮、总胆红素、降钙素原(PCT)、C反应蛋白(CRP)、PLR、血清hs-cTnT、GLU水平显著高于预后良好组(P＜0.05)。PLR、hs-cTnT、GLU水平与CTSI评分均呈显著正相关(P＜0.05)。症状严重程度、CTSI评分、血清AMY、PCT、CRP、PLR、hs-cTnT、GLU是AP患者预后的危险因素(P＜0.05)。PLR、hs-cTnT、GLU对AP患者预后均具有一定的预测价值,但联合预测价值高于单独预测价值。 结论PLR、hs-cTnT、GLU水平与AP患者病情严重程度呈明显相关性,且对AP患者预后均具有一定的预测价值,但其联合检测的预测价值高于单独指标预测价值。 |

| AimTo analyze the predictive value of combined detection of platelet to lymphocyte ratio (PLR), high-sensitivity cardiac troponin T (hs-cTnT) and blood glucose (GLU) on the prognosis in patients with acute pancreatitis (AP). MethodsThe data of 157 patients with AP were retrospectively analyzed. According to the prognosis status, the patients were divided into good prognosis group (135 cases) and poor prognosis group (22 cases). The general data, PLR, serum hs-cTnT and GLU levels were compared between the two groups. Pearson correlation coefficient was used to analyze the correlation between PLR, hs-cTnT and GLU levels and CT severity index (CTSI) score. Logistic regression analysis was applied to analyze the influencing factors affecting the prognosis of patients. ROC curve was adopted to analyze the evaluation value of combination of PLR, hs-cTnT and GLU on the prognosis in patients with AP. ResultsThe proportions of severe AP patients and CTSI≥3 points, body temperature, white blood cell count, levels of amylase (AMY), serum creatinine, blood urea nitrogen, total bilirubin, procalcitonin (PCT), C-reactive protein (CRP), PLR, serum hs-cTnT and GLU in poor prognosis group were significantly higher than those in good prognosis group (P＜0.05). PLR, hs-cTnT and GLU were significantly positively correlated with CTSI score (P＜0.05). Severity of symptoms, CTSI score and levels of serum AMY, PCT, CRP, PLR, hs-cTnT and GLU were the risk factors affecting the prognosis in AP patients (P＜0.05). PLR, hs-cTnT and GLU all had certain predictive value on AP patients, but the value of combined prediction was higher than that of single prediction. ConclusionThe PLR, hs-cTnT and GLU are significantly correlated with the disease severity of AP patients, and have certain predictive value on the prognosis of patients, but the predictive value of combined detection is higher than that of single indicator. |
