| 目的分析臂踝脉搏波传导速度(baPWV)、血浆骨形态发生蛋白4(BMP4)对高血压患者发生高血压心肌病(HHD)的预测价值。 方法193例原发性高血压患者根据有无HHD,将其分为高血压组(单纯高血压)126例和HHD组(高血压合并HHD)67例。检测两组患者入院时baPWV和血浆BMP4水平。比较两组临床资料及心脏超声相关指标。采用多因素Logistic回归分析高血压患者发生HHD的影响因素,ROC曲线评估baPWV、血浆BMP4水平对高血压患者发生HHD的预测价值。 结果与高血压组比较,HHD组年龄、体质指数、高血压病程、收缩压、超敏C反应蛋白(hs-CRP)、心脏超声相关指标、baPWV、血浆BMP4水平均显著升高(P＜0.05),而舒张压降低(P＜0.05)。多因素Logistic回归分析结果显示,hs-CRP、baPWV、血浆BMP4是原发性高血压患者发生HHD的独立危险因素。ROC曲线结果显示,baPWV、血浆BMP4联合预测效能高于单独预测效能。 结论baPWV、血浆BMP4水平对高血压发生HHD具有一定的预测价值。 |

| AimTo analyze the predictive value of brachial-ankle pulse wave velocity (baPWV) and plasma bone morphogenetic protein 4(BMP4) levels for the occurrence of hypertensive heart diseases (HHD) in hypertensive patients. MethodsA total of 193 patients with primary hypertension were divided into the hypertension group (simple hypertension) of 126 cases and the HHD group (hypertension combined with HHD) of 67 cases according to the presence or absence of HHD. baPWV and plasma BMP4 levels at the hospitalization of the two groups of patients were detected. The clinical data and echocardi-related indicators of the two groups were compared. Multivariate Logistic regression analysis was used to identify effectival factors for HHD in hypertensive patients. ROC curve analysis was performed to assess the predictive value of baPWV and plasma BMP4 levels for the development of HHD in hypertensive patients. ResultsCompared with the hypertension group, the HHD group showed significantly higher age, body mass index, duration of hypertension, systolic blood pressure, hypersensitivity C-reactive protein (hs-CRP), echocardiographic parameters, baPWV, and plasma BMP4 levels (P＜0.05), while diastolic blood pressure was lower (P＜0.05). Multivariate Logistic regression analysis indicated that hs-CRP, baPWV, and plasma BMP4 were independent risk factors for HHD in patients with essential hypertension. ROC curve analysis demonstrated that the combined predictive value of baPWV and plasma BMP4 was higher than that of each marker alone. ConclusionbaPWV and plasma BMP4 levels have predictive value for the development of HHD in hypertensive patients. |
