目的探究隐形矫治器矫治与传统固定矫治治疗重度牙周炎正畸患者牙槽骨改建的效果观察。 方法将134例重度牙周炎正畸患者随机均分为传统组与隐形组,其中传统组采用传统固定矫治器矫治,隐形组采用无托槽隐形矫治器矫治。比较两组牙周健康指标、唇倾度、牙槽骨高度变化及密度变化、美容效果评分及并发症发生率。 结果正畸前后,两组GI、SPD与GBI均较牙周治疗前下降(P<0.05),正畸12个月后,传统组GI、SPD与GBI较正畸前均上升(P<0.05),且隐形组正畸12个月后低于传统组(P<0.05);正畸12个月后,两组上、下颌唇倾度较正畸前均降低,且隐形组低于传统组(P<0.05),传统组上、下牙槽骨高度较正畸前降低(P<0.05),且隐形组高于传统组(P<0.05),传统组牙槽骨密度较正畸前下降(P<0.05),牙槽嵴顶舌侧骨密度高于传统组(P<0.05);两组患者美容效果评分较正畸前上升,且隐形组高于传统组(P<0.05)。两组并发症发生率差异无显著性(P>0.05)。 结论无托槽隐形矫治器用于重度牙周炎患者正畸治疗,对牙周及牙槽骨影响小,整体美观度高,值得推荐。 |

AimTo investigate the effects of invisible orthodontic treatment and traditional fixed orthodontic treatment on alveolar bone remodeling in patients with severe periodontitis. Methods134 periodontitis orthodontic patients were randomly divided into a traditional group and an invisible group, respectively. The traditional group was treated with traditional fixed orthodontic appliances, while the invisible group was treated with non-bracket invisible orthodontic appliances. The periodontal health indicators, lip inclination, changes in alveolar bone height and density, cosmetic effect scores, and incidence of complications between two groups were compared. ResultsThe GI, SPD, and GBI of both groups before and after orthodontic treatment decreased compared with that before periodontal treatment (P<0.05). After 12 months of orthodontic treatment, the GI, SPD, and GBI of the traditional group increased compared with that before orthodontic treatment (P<0.05), and the invisible group was lower than the traditional group after 12 months of orthodontic treatment (P<0.05); After 12 months of orthodontic treatment, the inclination of the upper and lower jaw lips in both groups decreased compared with that before orthodontic treatment, and the invisible group was lower than the traditional group (P<0.05). The height of the upper and lower alveolar bones in the traditional group decreased compared with that before orthodontic treatment (P<0.05), and the invisible group was higher than the traditional group (P<0.05). The alveolar bone density in the traditional group decreased compared with that before orthodontic treatment (P<0.05), and the bone density on the lingual side of the alveolar ridge was higher than that in the traditional group (P<0.05); The cosmetic effect scores of the two groups of patients increased compared with that before orthodontic treatment, and the invisible group was higher than the traditional group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). ConclusionThe use of non-bracket invisible aligners for orthodontic treatment of severe periodontitis patients has minimal impact on the periodontal and alveolar bone, and overall high aesthetic appeal, which is worthy of recommendation. |
