目的探讨胸部放射学严重程度指数(RSI)对人副流感病毒(HPIV)相关下呼吸道感染(LRTI)患儿的短期预后价值。 方法回顾性收集HPIV相关LRTI患儿80例,按照短期(7天)预后分为预后不良组(n=21)和预后良好组(n=59)。比较两组C反应蛋白(CRP)、中性粒细胞/淋巴细胞比值(NLR)、RSI。分析患儿预后不良的影响因素,以及CRP、NLR和RSI对预后不良的预测效力。结果与预后良好组比较,预后不良组CRP升高,NLR降低,RSI得分升高(P<0.05)。相关性分析结果表明,RSI得分与CRP呈正相关,与NLR呈负相关(P<0.001)。CRP、NLR和RSI是患儿短期预后不良的独立影响因素(P<0.05)。ROC分析结果显示,RSI、CRP、NLR对患儿短期预后均具有良好的预测价值,3指标联合时预测效能最高(P<0.05)。 结论RSI对于HPIV相关LRTI患儿短期预后有一定预测效能,将其与血液学指标联用可提高其预测的应用价值。 |

AimTo investigate the short-term prognostic value of chest radiological severity index (RSI) in children with human parainfluenza virus (HPIV)-related lower respiratory tract infections (LRTI). MethodsA total of 80 children with HPIV-related LRTI were retrospectively collected and divided into poor prognosis group (n=21) and good prognosis group (n=59) according to the short-term (7 days) prognosis. C-reactive protein (CRP), neutrophil / lymphocyte ratio (NLR), RSI were compared between the two groups. The influencing factors of poor prognosis and the predictive effects of CRP, NLR and RSI on poor prognosis were analyzed. ResultsCompared with the good prognosis group, CRP increased, NLR decreased, and RSI score increased in the poor prognosis group (P<0.05). Correlation analysis showed that RSI score was positively correlated with CRP and negatively correlated with NLR (P<0.001). CRP, NLR and RSI were independent influencing factors of poor short-term prognosis in children (P<0.05). The results of ROC analysis showed that RSI, CRP and NLR had predictive value for the short-term prognosis of children, and the predictive efficacy was the highest when the three indexes were combined (P<0.05). ConclusionRSI has a certain predictive value on the short-term prognosis of children with HPIV-related LRTI, and its application value can be improved by combining it with hematological indicators. |
