| 目的探讨振幅整合脑电图对感染新生儿化脓性脑膜炎早期诊断的作用以及预后的预测价值。 方法收集确诊为感染的新生儿52例,确诊后24 h内根据振幅整合脑电图(aEEG)检测结果分为正常组、轻度异常组、重度异常组。比较各组出生6个月时贝利婴儿发育量表评估及出生12个月时Gesell发育量表评分。以脑脊液生化检查作为评估金标准,判断感染的新生儿52例中化脓性脑膜炎例数。ROC分析aEEG对新生儿化脓性脑膜炎的诊断效能。 结果患儿在出生6个月时进行贝利婴儿发育量表评估31例正常,11例过渡,10例异常,贝利婴儿发育量表评估结果在不同程度aEEG组间差异存在统计学意义(P＜0.05)。aEEG异常患儿的Gesell发育量表3项检测值均较aEEG正常患儿低(P＜0.05)。感染新生儿52例中有10例患儿存在化脓性脑膜炎。ROC分析显示,aEEG诊断新生儿化脓性脑膜炎具有良好的诊断效能。 结论aEEG对感染性新生儿化脓性脑膜炎的早期诊断及预后的预测具有重要的意义。 |

| AimTo investigate the effects of amplitude integrated aEEG on the early diagnosis of infected neonatal purulent meningitis. Methods52 newborns diagnosed with infection were collected, and divided into normal, mild abnormal and severe abnormal based on aEEG background activity within 24 h after diagnosis. Pele infant development scale at 6 months of birth and Gesell development scale at 12 months of birth were compared in each group. Cerebrospinal fluid biochemistry was used as the gold standard for evaluation, to determine the number of cases of purulent meningitis among 52 infected newborns. ROC was used to analyze the diagnostic efficacy of aEEG on infected neonatal purulent meningitis. ResultsThe infants were assessed as 31 normal, 11 transition, and 10 abnormal at 6 months of birth based on Pele infant development scale, and the scores were significantly different among different groups with different aEEG levels (P＜0.05). The 3 parameters in Gesell development scores in children with abnormal aEEG were lower than those with normal aEEG (P＜0.05). Pyogenic meningitis was present in 10 children out of 52 infected neonates. The ROC curve analysis showed that the aEEG had good diagnostic efficacy for detecting neonatal suppurative meningitis. ConclusionThe early diagnosis and prognosis prediction of infectious neonatal purulent meningitis by aEEG are of great significanc. |
