目的观察益气复脉法联合耐力运动训练对射血分数保留型心力衰竭(HFpEF)患者心肺运动试验参数及血清N末端B型脑钠肽前体(NT-proBNP)、可溶性生长刺激表达基因2蛋白(sST2)的影响。 方法选取95例HFpEF患者,分为训练组(常规用药+耐力运动训练)和益气复脉组(常规用药+耐力运动训练+益气复脉法)。比较两组心肺运动试验参数、心率变异性、心脏彩超指标及血清NT-proBNP、sST2、同型半胱氨酸(Hcy)的差异。 结果与治疗前比较,两组峰值氧耗量、无氧代谢阈值耗氧量、相邻NN均方差、NN50占NN间期百分率、舒张早期二尖瓣环峰值速度(e′)均升高,且益气复脉组高于训练组(P<0.05);两组左心房容积指数、左心室舒张早期二尖瓣血流最大速度/e′比值、分钟通气量/二氧化碳排出量斜率、左心室质量指数、血清NT-proBNP、sST2、Hcy均降低,且益气复脉组低于训练组(P<0.05)。 结论益气复脉法联合耐力运动训练可改善HFpEF患者左心室舒张功能,降低血清NT-proBNP、sST2水平,改善心肺运动耐力和心率变异性。 |

AimTo observe the effects of Yiqi-pulse restoration method combined with endurance exercise training on cardiopulmonary exercise test parameters and serum N-terminal B-type brain natriuretic peptide precursor (NT-proBNP) and soluble growth-stimulated expressed gene 2 protein (sST2) in patients with heart failure with preserved ejection fraction (HFpEF). Methods95 patients with HFpEF were selected and divided into the training group (conventional medication+endurance exercise training) and the Yiqi-pulse rejuvenation group (conventional medication+endurance exercise training+Yiqi-pulse rejuvenation method). The differences in cardiopulmonary exercise test, heart rate variability, cardiac ultrasound indexes and serum NT-proBNP, sST2 and homocysteine (Hcy) were compared between the two groups. ResultsCompared with the pre-treatment, peak oxygen consumption, anaerobic threshold, mean squared deviation of adjacent NNs, percentage of NN50 to NN interval, and peak mitral annular velocity in early diastole were increased in both groups, and these parameters were higher in the Yiqi-pulse rejuvenation group compared with the training group (P<0.05). Left atrial volume index, ratio of maximal velocity of mitral inflow in early diastole in the left ventricle to the peakmitral annular velocity in early diastole, slope of minute ventilation to carbon dioxide output, and left ventricular mass index were increased in the two groups, serum NT-proBNP, sST2, and Hcy were all reduced, and the Yiqi-pulse rejuvenation group was lower than the training group (P<0.05). ConclusionThe Yiqi-pulse restoration method combined with endurance exercise training improves left ventricular diastolic function, reduces serum NT-proBNP and sST2 levels, and improves cardiopulmonary exercise tolerance and heart rate variability in patients with HFpEF. |
