目的探讨胎儿心脏测量参数Z-评分在筛查胎儿心脏结构畸形方面的临床应用价值。 方法选取产前筛查的93例心脏结构异常的胎儿病例,根据其心脏结构异常表现分为右心发育不良组(n=48)和左心发育不良组(n=45例),选取同期筛查的45例正常胎儿为对照组,以胎儿双顶径(BPD)、股骨长径(FL)、孕周(GA)等生物学生长参数为自变量,各心脏测量参数主动脉瓣(AO)、升主动脉(AAO)、肺动脉瓣(PA),左肺动脉(LPA)、右肺动脉(RPA)内径为因变量,建立各心脏测量参数Z-评分,比较各组胎儿Z-评分差异,ROC曲线分析对比各心脏测量参数Z-评分对心脏结构畸形的诊断效能。 结果与对照组比较,右心发育不良组PA、LPA和RPA-Z值下降,AO、AAO-Z值上升(均P<0.05),左心发育不良组PA、LPA和RPA-Z值上升,AO、AAO-Z值下降(均P<0.05)。与左心发育不良组比较,右心发育组PA、LPA和RPA-Z值下降,AO、AAO-Z值上升(均P<0.05)。ROC曲线特征显示,AO、AAO、PA、LPA、RPA的Z-评分均具有诊断胎儿心脏结构畸形的效能(均P<0.05),其中左心发育不良以AO-Z-评分诊断价值最高,右心发育不良以PA-Z-评分诊断价值最高(P<0.05)。 结论胎儿主动脉和肺动脉超声Z-评分可评估胎儿心脏结构生长发育情况,可为临床产前胎儿心脏结构畸形筛查提供临床指导。 |

AimTo explore the clinical application value of fetal heart measurement parameter Z-score in screening fetal heart structural malformations. MethodsA total of 93 fetuses with heart structural malformations were enrolled. According to different malformations, fetuses were divided into right heart dysplasia group (n=48) and left heart dysplasia group (n=45). A total of 45 normal fetuses during the same period were enrolled as control group. Fetal biological growth parameters such as biparietal diameter (BPD), femur length (FL) and gestational age (GA) were selected as independent variables, and heart measurement parameters such as diameters of aortic valve (AO), ascending aorta (AAO), pulmonary valve (PA), left pulmonary artery (LPA) and right pulmonary artery (RPA) were selected as dependent variables, to conduct heart measurement parameter Z-score. The differences in fetal Z-score among all groups were compared. The diagnostic efficiency of different heart measurement parameter Z-score for heart structural malformations was analyzed and compared by ROC curves. ResultsCompared with control group, PA, LPA and RPA-Z were decreased, while AO and AO-Z were increased in right heart dysplasia group (P<0.05), PA, LPA and RPA-Z were increased, while AO and AO-Z were decreased in left heart dysplasia group (P<0.05). Compared with left heart dysplasia group, PA, LPA and RPA-Z were decreased, while AO and AAO-Z were increased in right heart dysplasia group (P<0.05). ROC curve characteristics showed that Z-scores of AO, AAO, PA, LPA and RPA were all of diagnostic efficiency for fetal heart structural malformations (P<0.05). The diagnostic value of AO-Z-score was the highest for left heart dysplasia, while diagnostic value of PA-Z-score was the highest for right heart dysplasia (P<0.05). ConclusionUltrasound Z-scores of fetal aorta and pulmonary artery can evaluate the growth and development of fetal heart structure, which can provide clinical guidance for prenatal screening of fetal heart structure malformations. |
