      目的分析2型糖尿病(T2DM)患者血清同型半胱氨酸(Hcy)、促甲状腺激素(TSH)水平与其糖尿病肾脏病变(DKD)的相关性。 方法选取88例T2DM患者作为研究对象,根据患者是否发生肾脏病变将其分为无DKD组、轻度DKD组和重度DKD组。比较各组临床资料和血清Hcy、TSH水平。采用Pearson相关性分析血清Hcy、TSH水平与尿微量白蛋白与肌酐比值(ACR)的相关性。Logistic回归分析T2DM患者发生肾脏病变的影响因素。ROC评价血清Hcy、TSH单独及联合检测对DKD的评估价值。 结果重度DKD组年龄大于轻度DKD组和无DKD组(P<0.05),轻度DKD组年龄大于无DKD组(P<0.05);重度DKD组收缩压(SBP)、ACR、血清Hcy、TSH水平均高于轻度DKD组和无DKD组(P<0.05),轻度DKD组SBP、ACR、血清Hcy、TSH水平均高于无DKD组(P<0.05)。T2DM患者血清Hcy、TSH水平与ACR均呈正相关(P<0.05)。Logistic多因素分析结果显示,年龄、SBP、Hcy、TSH均是T2DM患者发生肾脏病变的危险因素(P<0.05)。ROC曲线分析结果显示,血清Hcy、TSH联合检测T2DM患者肾脏病变发生的AUC高于单独检测值。 结论T2DM患者血清Hcy、TSH水平是肾脏病变的影响因素。

      AimTo analyze the correlation between serum homocysteine (Hcy) and thyroid stimulating hormone (TSH) levels and diabetic kidney disease (DKD) in patients with type 2 diabetes mellitus (T2DM). Methods88 T2DM patients in our hospital were selected as the study objects, and were divided into non-DKD group, mild DKD group and severe DKD group according to whether or not the patients had kidney lesions. The clinical data and serum Hcy and TSH levels of all groups were compared. Pearson correlation was used to analyze the correlation between serum Hcy and TSH levels and urinary microalbumin to creatinine ratio (ACR). Logistic regression analysis was conducted to analyze the influencing factors of kidney lesions in T2DM patients. ROC was used to evaluate the value of serum Hcy and TSH alone and in combination for DKD. ResultsThe age of severe DKD group was higher than that of mild DKD group and non-DKD group (P<0.05), and the age of mild DKD group was higher than that of non-DKD group (P<0.05).Systolic blood pressure (SBP), ACR, serum Hcy and TSH levels in severe DKD group were higher than those in mild DKD group and non-DKD group (P<0.05), and SBP, ACR, serum Hcy and TSH levels in mild DKD group were higher than those in non-DKD group (P<0.05). Serum Hcy and TSH levels were positively correlated with ACR in T2DM patients (P<0.05). Logistic multivariate analysis showed that age, SBP, Hcy and TSH were independent risk factors for renal lesions in T2DM patients (P<0.05). ROC curve analysis showed that the AUC of serum Hcy and TSH after combined detection in T2DM patients was higher than that after single dection. ConclusionSerum Hcy and TSH levels in T2DM patients are the influencing factors of renal lesions.
