      目的探讨无痛胃肠镜检查患者发生肌阵挛的危险因素及丙泊酚预处理的效果。 方法回顾性选择行依托咪酯无痛胃肠镜检查患者152例,COX模型分析肌阵挛发生的危险因素。另前瞻性选择行无痛胃肠镜检查患者92例,分为对照组(依托咪酯)和丙泊酚低、中、高剂量组(0.3、0.6和0.8 mg/kg),比较各组发生肌阵挛和不良反应情况。 结果152例胃肠镜检查患者中肌阵挛发生率为75%。COX模型分析发现,年龄、是否初次检查是无痛胃肠镜检查肌阵挛发生的独立危险因素。与对照组和低剂量组比较,中剂量组和高剂量组肌阵挛发生率降低;与对照组比较,低、中、高剂量组不良反应发生率降低(P<0.05)。 结论年龄、是否初次检查是无痛胃肠镜检查肌阵挛发生的独立危险因素,中、高剂量丙泊酚预处理可减少肌阵挛。

      AimTo investigate the risk factors of myoclonus and the effect of preconditioning with propofol in painless gastroenteroscopy. MethodsThe risk factors of myoclonus were analyzed by COX model in 152 patients who underwent etomidate painless gastroenteroscopy. In addition, 92 patients underwent painless gastroenteroscopy were divided into control group and propofol low, medium and high dose groups (0.3,0.6 and 0.8 mg/kg) to compare the myoclonus and adverse reactions in different groups. ResultsThe incidence of myoclonus in 152 gastroenteroscopy patients was 75%.COX model analysis showed that age and initial examination were independent risk factors for myoclonus in painless gastroenteroscopy. Compared with control group and low dose group, the incidence of myoclonus in medium dose group and high dose group was decreased, and the incidence of adverse reactions in low, medium and high dose group was decreased compared with control group (P<0.05). ConclusionAge and initial examination were independent risk factors for myoclonus in painless gastroenteroscopy. Preconditioning with medium and high doses of propofol could reduce myoclonus.
