      目的研究早产儿出生早期血清高敏C-反应蛋白(hs-CRP)、核因子-κB(NF-κB)与脑损伤的关系。 方法选取早产儿185例,根据是否发生脑损伤分为非脑损伤组和脑损伤组。将脑损伤患儿根据不同脑损伤程度分为轻度组、中度组和重度组。采用Spearman相关分析、Logistic回归分析及ROC分析血清hs-CRP、NF-κB水平与脑损伤严重程度的关系及对早产儿脑损伤的预测价值。 结果脑损伤组血清hs-CRP、NF-κB水平高于非脑损伤组(P<0.05)。患儿血清hs-CRP、NF-κB水平重度组、中度组高于轻度组,重度组高于中度组(P<0.05)。血清hs-CRP、NF-κB水平与脑损伤严重程度呈正相关(P<0.05)。Logistic回归分析显示,产前应用糖皮质激素为早产儿脑损伤的保护因素,妊娠期高血压、妊娠期糖尿病、出生1 minApgar评分及血清hs-CRP、NF-κB水平均为危险因素(P<0.05)。ROC曲线显示,血清hs-CRP、NF-κB及二者联合判断早产儿脑损伤的曲线下面积(AUC)分别为0.852、0.856、0.902(P<0.05)。 结论脑损伤早产儿血清hs-CRP、NF-κB水平异常升高,hs-CRP、NF-κB为影响脑损伤发生的危险因素,二者联合预测早产儿脑损伤的价值较高。

      AimTo study the relationship between serum high sensitivity C-reactive protein (hs-CRP) and nuclear factor κB (NF-κB) levels and brain injury in premature infants. Methods185 preterm infants were selected to analyze the relationship between the level of serum hs-CRP and NF-κB and the severity of brain injury, in order to analyze the predictive value of brain injury in premature infants. ResultsThe levels of hs-CRP and NF-κB in brain injury group were higher than those in non-brain injury group (P<0.05). The levels of serum hs-CRP and NF-κB in severe, moderate and mild groups were decreased successively (P<0.05). Serum hs-CRP and NF-κB levels were positively correlated with the severity of brain injury (P<0.05). Logistic regression analysis showed that prenatal use of glucocorticoids was a protective factor for brain injury in preterm infants, whereas gestational hypertension, gestational diabetes, Apgar score 1 min after birth and serum hs-CRP and NF-κB levels were all risk factors (P<0.05). ROC curve showed that the area under the curve (AUC) of serum hs-CRP, NF-κB and their combination to determine brain injury in preterm infants were 0.852,0.856 and 0.902, respectively (P<0.05). ConclusionSerum hs-CRP and NF-κB levels are abnormally elevated in premature infants with brain injury, and are risk factors affecting the occurrence of brain injury. Furthermore, the combination of them has high value in predicting brain injury in premature infants.
