目的探讨口腔正畸患者牙周疾病的影响因素及骨保护蛋白(OPG)、炎症因子、菌斑指数(PLI)对其的预测价值。 方法选择本院行口腔正畸的106例患者,根据患者是否患有牙周病分为牙周病组47例及对照组59例。比较两组患者牙周龈沟液中核因子-κB受体活化因子配体(RANKL)、OPG、RANKL/OPG、炎症因子水平及牙周状况。采用Logistics回归分析患者发生牙周病的影响因素,采用ROC分析各指标对牙周病发生的预测价值。 结果治疗后,两组患者较治疗前RANKL、RANKL/OPG、C-反应蛋白(CRP)、白细胞介素(IL)-1β、IL-6、牙龈指数(GI)、出血指数(SBI)及PLI水平均升高,OPG水平下降(P<0.05);但牙周病组RANKL、RANKL/OPG、CRP、IL-1β、IL-6及PLI水平较对照组高,OPG水平较对照组低(P<0.05)。OPG、CRP、IL-1β及PLI为患者发生牙周病的影响因素。OPG、CRP、IL-1β及PLI均对预测患者发生牙周疾病具有较高的灵敏度及特异度,且联合诊断价值最高。 结论OPG、CRP、IL-1β及PLI水平为患者牙周病发生的影响因素,且联合诊断价值最高。 |

AimTo explore the influencing factors of periodontal disease in orthodontic patients and the predictive value of osteoprotegerin (OPG), inflammatory factors and plaque index (PLI) on periodontal disease in orthodontic patients. MethodsA total of 106 patients undergoing orthodontic treatment in our hospital were divided into periodontal disease group (47 cases) and control group (59 cases). The levels of nuclear factor-KB receptor activating factor ligand (RANKL), OPG, RANKL/OPG, inflammatory factors and periodontal indexes in gingival fluid of the two groups were compared. Logistics regression was used to analyze the related factors affecting the occurrence of periodontal disease in patients, and ROC was used to test the predictive value of each index to the occurrence of periodontal disease. ResultsAfter treatment, the levels of RANKL, RANKL/OPG, C-reactive protein (CRP), interleukin (IL)-1β, IL-6, gum index (GI), bleeding index (SBI) and PLI were increased in both groups, while the levels of OPG were decreased; However, the levels of RANKL, RANKL/OPG, CRP, IL-1β, IL-6 and PLI in periodontal disease group were higher than those in control group, and the levels of OPG were lower than those in control group (P<0.05). OPG, CRP, IL-1β and PLI were the factors affecting the occurrence of periodontal disease. OPG, CRP, IL-1β and PLI all had high sensitivity and specificity in predicting the occurrence of periodontal disease in patients, and had the highest combined diagnostic value. ConclusionThe levels of OPG, CRP, IL-1β and PLI are the factors affecting the occurrence of periodontal disease, and the combined diagnosis value is the highest. |
