目的探讨血清肿瘤标志物神经元特异性烯醇化酶(NSE)、细胞角蛋白19可溶性片段(CYFRA21-1)、癌胚抗原(CEA)水平,及血清骨代谢指标Ⅰ型胶原羧基末端肽(ICTP)、Ⅰ型胶原吡啶交联氨基末端肽(NTx)、血清骨特异性碱性磷酸酶(BALP)与肺癌骨转移的相关性。 方法选取发生肺癌骨转移(转移组)、无骨转移组(未转移组)及健康志愿者(对照组),各组60例。检测3组肿瘤标志物及骨代谢(BALP、ICTP、NTx)水平。根据ROC曲线评估肺癌骨转移模型的预测效能。 结果转移组NTx、ICTP、BALP、NSE、Cyfra21-1CEA水平较未转移组高,且未转移组高于对照组(P<0.05);转移组的6项指标阳性率较未转移组高(P<0.05);CEA、NSE水平在不同组织分型中差异有统计学意义(P<0.05),CEA在腺癌中最高,NSE在小细胞肺癌中最高(P<0.05);随着肺癌分期的增加,NTx、ICTP、BALP、CEA、Cyfra21-1水平呈递增趋势(P<0.05);NTx、ICTP、CEA、NSE与骨转移分级呈正相关(均P<0.05)。6项标志物联合预测模型预测肺癌骨转移的AUC(95%CI)为0.934(0.879~0.970),高于单项指标预测效能。 结论血清骨代谢指标(NTx、ICTP、BALP)及肿瘤标志物(CEA、Cyfra21-1、NSE)水平与肺癌患者发生骨转移具有一定的相关性,且6项联合检测预测肺癌骨转移的效能较高。 |

AimTo investigate the correlation between levels of serum neuron-specific enolase (NSE), cytokeratin 19 soluble fragment (CYFRA21-1), carcinoembryonic antigen (CEA), serum bone metabolism indexes of type Ⅰ collagen carboxyl terminal peptide (ICTP), type Ⅰ collagen pyridine cross-linked amino terminal peptide (NTx), serum bone specific alkaline phosphatase (BALP) and bone metastasis in lung cancer patients. Methods180 cases each were divided into 3 groups, including the group with bone metastases of lung cancer (metastatic group), without bone metastases (non-metastatic group) and healthy volunteers (control group), with 60 cases in each group. The levels of tumor markers and bone metabolism (BALP, ICTP, NTX) in the subjects and the positive rates of the above-mentioned markers in the patients were determined. The predictive performance of the established lung cancer bone metastasis model was assessed according to the ROC curve. ResultsMetastasis group had higher levels of NTX, ICTP, BALP, NSE, and cyfra21-1cea than those without metastasis, and the non-metastatic group was higher than the control group (P<0.05); The positive rates of the six indicators were higher in the metastasis group than in the non-metastasis group (P<0.05); The levels of CEA and NSE were significantly different among different tissue types (P<0.05), with CEA levels highest in adenocarcinoma, and NSE levels highest in SCLC (P<0.05); With the stage of lung cancer increased, the levels of NTX, ICTP, BALP, CEA, and CYFRA21-1 showed an increasing trend (P<0.05); NTX, ICTP, CEA and NSE were positively correlated with the grade of bone metastasis in lung cancer patients (all P<0.05). The combined prediction model of 6 markers gave an AUC of 0.934 (95%CI 0.879-0.970), higher than the predictive efficacy of the individual markers in predicting bone metastases in lung cancer. ConclusionSerum levels of bone metabolism markers (NTX, ICTP, BALP) and tumor markers (CEA, CYFRA21-1, NSE) were correlated with the occurrence of bone metastasis in lung cancer patients, and the combination of the six tests is highly effective in predicting bone metastasis in lung cancer. |
