      目的分析胸腔积液癌细胞表皮生长因子受体(EGFR)基因突变与非小细胞肺癌(NSCLC)患者外周血中性粒细胞/淋巴细胞比值(NLR)、淋巴细胞/单核细胞比值(LMR)水平的关系。 方法选取82例NSCLC患者,采用Sanger测序法检测患者胸腔积液癌细胞EGFR基因突变,分析其与临床特征的关系。检测外周血淋巴细胞计数、中性粒细胞计数、单核细胞计数水平,并计算NLR、LMR,分析EGFR基因突变与NLR、LMR的关系。 结果82例NSCLC患者中胸腔积液癌细胞出现EGFR突变共24例(29.27%),EGFR基因突变与肿瘤位置有关系,周围型者EGFR基因突变率较中央型高(P<0.05)。EGFR基因突变型者外周血NLR低于野生型者,LMR高于野生型者(P<0.05)。ROC曲线分析显示,NLR低水平、LMR高水平者胸腔积液癌细胞EGFR基因突变率较高(P<0.05)。 结论NSCLC外周血NLR低水平、LMR高水平患者更易发生胸腔积液癌细胞EGFR基因突变。

      AimTo analyze the relationship between gene mutation of epidermal growth factor receptor (EGFR) in pleural effusion cancer cells and peripheral blood neutrophil/lymphocyte ratio (NLR) and lymphocyte/monocyte ratio (LMR) in patients with non-small cell lung cancer (NSCLC). MethodsEighty-two patients with NSCLC were selected and the EGFR gene mutation in pleural effusion cancer cells was detected by Sanger sequencing method to analyze the relationship with clinical features. The peripheral blood lymphocyte count, neutrophil count and monocyte count were detected, NLR and LMR were calculated. The relationship between EGFR gene mutation and NLR, LMR were analyzed. ResultsThere were 24 cases (29.27%) with EGFR gene mutation in pleural effusion cancer cells of the 82 cases of NSCLC. EGFR gene mutation was correlated with tumor sites. The incidence of EGFR gene mutation in patients with peripheral NSCLC was higher than that with central NSCLC (P<0.05). The peripheral blood NLR in patients with mutation gene was lower than that with wild gene, while LMR was greater than that with wild gene (P<0.05). ROC curves analysis showed that incidence of EGFR gene mutation was higher in patients with low-level NLR and high-level LMR (P<0.05). ConclusionThe incidence of EGFR gene mutation is higher in NSCLC patients with low-level NLR and high-level LMR.
