| 目的比较Gamma髓内钉与InterTan髓内钉两种不同内固定方式对老年股骨转子间骨折的治疗效果。 方法回顾性选择老年外侧壁不稳定型股骨转子间骨折患者102例,根据治疗方式不同分为Gamma组(n＝50)和InterTan组(n＝52),比较两组患者手术相关指标、骨折愈合相关指标、术后髋关节功能恢复情况以及头钉尖顶距(TAD)及术后并发症的发生。 结果InterTan组手术时间长于Gamma组(P＜0.05),出血量多于Gamma组(P＜0.05)；InterTan组术后3个月Harris髋关节功能评分优良率高于Gamma组,但差异无统计学意义(P＞0.05)；术后3个月,Gamma组TAD大于InterTan组(P＜0.05)；InterTan组术后并发症发生率低于Gamma组,但差异无统计学意义(P＞0.05)。 结论Gamma髓内钉与InterTan髓内钉两种内固定方式都适用于治疗老年外侧壁不稳定型股骨转子间骨折,其中InterTan髓内钉内固定的稳定性更好,适用于严重骨质疏松或者粉碎性骨折患者,而Gamma髓内钉内固定更加适用于对手术不耐受的患者。 |

| AimTo compare the therapeutic effects of two different internal fixation methods of Gamma intramedullary nail and InterTan intramedullary nail on elderly patients with femoral intertrochanteric fractures. MethodsA total of 102 elderly patients with unstable lateral femoral intertrochanteric fractures were retrospectively selected and divided into Gamma group (n＝50) and InterTan group (n＝52) according to different treatment methods. The surgery-related indicators, fracture healing-related indicators, postoperative hip joint function recovery and tip apex distance (TAD) were compared between the two groups, and the occurrence of postoperative complications were observed in the two groups. ResultsThe operative time in InterTan group was longer than that in Gamma group (P＜0.05), and the bleeding volume was more than that in Gamma group (P＜0.05). The excellent and good rate of Harris hip joint function score in InterTan group at 3 months after surgery was higher than that in Gamma group (P＞0.05). At 3 months after surgery, TAD in Gamma group was longer than that in InterTan group (P＜0.05). The incidence rate of postoperative complications in InterTan group was lower than that in Gamma group (P＞0.05). ConclusionBoth Gamma intramedullary nail and InterTan intramedullary nail internal fixation are suitable for the treatment of elderly unstable lateral femoral intertrochanteric fractures. InterTan intramedullary nail internal fixation has better stability and is suitable for patients with severe osteoporosis or comminuted fractures while Gamma intramedullary nail internal fixation is more suitable for patients who are intolerant to surgery. |
