| 目的探讨蛋白C(PC)活性、D-二聚体(D-D)、血小板计数(PLT)与急性肺栓塞(APE)病情程度的关系及联合预测预后的价值。 方法选择APE患者160例(APE组),按照病情程度分为低危组58例、中危组59例与高危组43例；按照预后情况分为存活组128例与死亡组32例。另选择同期健康体检者100例作为对照组。比较各组血浆PC活性、D-D和PLT水平变化；分析PC活性、D-D和PLT与病情程度相关性及诊断APE预后价值。 结果各组PC活性和PLT、D-D水平存在差异(P＜0.05)。PC活性和PLT与病情程度呈负相关,D-D与病情程度呈正相关(P＜0.05)。经ROC曲线分析,PC活性、D-D、PLT三者联合诊断APE的灵敏度和特异度高于单独诊断。 结论APE患者血浆PC活性和PLT降低而D-D水平升高,且与病情严重程度密切相关；对预测预后具有良好诊断价值。 |

| AimTo investigate the relationship between protein C (PC) activity, D-dimer (D-D), platelet count (PLT) and the severity of acute pulmonary embolism (APE) and the value of joint prediction of prognosis. Methods160 APE patients were selected and divided into a low-risk group of 58 cases, a medium risk group of 59 cases, and a high-risk group of 43 cases according to the severity of the condition; According to the prognosis, there were 128 cases in the survival group and 32 cases in the death group. Another 100 healthy subjects in the same period were selected as the control group. The plasma PC activity, D-D and PLT levels in APE group and control group were compared with each other in different disease severity and prognosis. The correlation of PC activity, D-D and PLT with the course of disease and the prognostic value of APE were analyzed. ResultsThere were differences in PC activity, PLT and D-D levels among all groups (P＜0.05). Pearson analysis showed that PC activity and PLT were negatively correlated with the severity of the disease, D-D was positively correlated with the severity of the disease (P＜0.05). According to ROC curve analysis, the sensitivity and specificity of PC activity+D-D+PLT combined diagnosis of APE patients are higher than PC activity, D-D and PLT. ConclusionThe plasma PC activity and PLT of APE patients decrease while the D-D level increases, which is closely related to the severity of the disease; it has a good diagnostic value for predicting the prognosis. |
