      目的探讨利拉鲁肽和艾塞那肽对2型糖尿病(T2DM)患者血糖及胃肠道反应的影响。 方法选取T2DM患者93例,根据治疗方式分为利拉鲁肽组和艾塞那肽组。比较两组患者血糖、血脂、肾功能、胰岛功能、炎症因子水平,以及胃肠道反应发生情况。 结果与治疗前比较,治疗后两组空腹血糖(FBG)、餐后2 h血糖(2 hPBG)、糖化血红蛋白(HbA1C)、总胆固醇、甘油三酯、血尿素氮、血肌酐(SCr)降低,利拉鲁肽组高密度脂蛋白胆固醇(HDLC)升高、低密度脂蛋白胆固醇(LDLC)降低;治疗后利拉鲁肽组FBG、2 hPBG、HbA1c、LDLC、SCr低于艾塞那肽组,HDLC高于艾塞那肽组(P<0.05)。与治疗前比较,治疗后两组空腹胰岛素、胰岛素抵抗指数、空腹C肽、餐后2 h C肽、肿瘤坏死因子-α、C-反应蛋白、白细胞介素-6降低,且利拉鲁肽组低于艾塞那肽组;治疗后两组胰岛β细胞功能指数升高,且利拉鲁肽组高于艾塞那肽组(P<0.05)。利拉鲁肽组和艾塞那肽组胃肠道反应发生率间差异无显著性(P>0.05)。 结论利拉鲁肽较艾塞那肽具有更佳降糖效果,胃肠道反应相当,更利于胰岛β细胞功能恢复和预后改善。

      AimTo investigate the effects of liraglutide and exenatide on blood glucose and gastrointestinal reaction in patients with type 2 diabetes (T2DM). MethodsNinety three patients with T2DM were selected and divided into the liraglutide group and exenatide group according to the treatment mode. The levels of blood glucose, blood lipid, renal function, pancreatic islet function, inflammatory factors and gastrointestinal reaction were compared between the two groups. ResultsCompared with that before treatment, fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin, total cholesterol, triglycerides, blood urea nitrogen, and serum creatinine decreased in the two groups after treatment, and high-density lipoprotein cholesterol (HDLC) increased and low-density lipoprotein cholesterol (LDLC) decreased in the liraglutide group; After treatment, FBG, 2 hPBG, HbA1c, LDLC, and SCr in liraglutide group were lower than those in exenatide group, while HDLC was higher than that in exenatide group (P<0.05). After treatment, fasting serum insulin, HOMA-IR, fasting C-peptide, 2 h postprandial C-peptide, tumor necrosis factor-α, C-reactive protein (CRP) and interleukin-6 (IL-6) decreased in the two groups, and the level of liraglutide group were lower than those of exenatide group; After treatment, the HOMA-β in both groups increased, and the level of liraglutide group was higher than that of exenatide group (P<0.05). There was no significant difference in the incidence of gastrointestinal reactions between liraglutide group and exenatide group (P>0.05). ConclusionLilalutide has a better hypoglycemic effect than exenatide, with a similar gastrointestinal reaction, which is more beneficial to islets of Langerhans β Cell function recovery and prognosis improvement.
