| 目的比较宫颈扩张球囊和缩宫素对足月妊娠宫颈成熟和分娩结局的影响。 方法将122例孕足月产妇随机等分为球囊组(宫颈扩张球囊引产)和缩宫素组(缩宫素引产),比较两组的促宫颈成熟总有效率、分娩结局、凝血因子水平和分娩期间并发症。 结果球囊组引产后Bishop评分和促宫颈成熟总有效率高于缩宫素组(P＜0.05)。球囊组阴道分娩率、新生儿Apgar评分高于缩宫素组,剖宫产率、羊水污染率、引产到临产时间、产程时间均小于缩宫素组(P＜0.05)。治疗后3天缩宫素组凝血指标和产后出血量优于球囊组(P＜0.05)。 结论宫颈扩张球囊较缩宫素可更有效促足月妊娠宫颈成熟,改善分娩结局,缩宫素可有效预防产后出血,增强凝血功能,临床可根据实际情况单用或联用。 |

| AimTo compare the effect of cervical dilatation balloon and oxytocin on cervical maturation and delivery outcome in term pregnancy. Methods122 full-term pregnant women were randomly divided into balloon group (induced labor by cervical dilation balloon) and oxytocin group (induced labor by oxytocin), and the total effective rate, delivery outcome, coagulation factor level and complications during delivery were compared between the two groups. ResultsThe Bishop score and the total effective rate of cervical ripening in the balloon group were higher than those in the oxytocin group (P＜0.05). The vaginal delivery rate and neonatal Apgar score in the balloon group were higher than those in the oxytocin group, and the cesarean section rate, amniotic fluid contamination rate, the time from induction to labor, and the time of labor process were lower than those in the oxytocin group (P＜0.05). The coagulation index and postpartum hemorrhage in oxytocin group were better than those in balloon group 3 days after treatment (P＜0.05). ConclusionCompared with oxytocin, cervical dilation balloon can more effectively promote cervical maturity of term pregnancy and improve delivery outcome. Oxytocin can effectively prevent postpartum hemorrhage and enhance blood coagulation function. It can be used alone or in combination according to the actual situation. |
