目的探讨亚甲基四氢叶酸还原酶(MTHFR)C677T基因多态性与脑梗死中国缺血性卒中亚型(CISS)分型及脑侧支循环的关系。 方法选取缺血性脑梗死患者167例作为观察组,同时选取健康志愿者160例作为对照组,比较两组间以及不同CISS分型患者间MTHFR C677T基因型分布。比较不同MTHFR C677T基因型分布患者血清同型半胱氨酸(Hcy)差异,并分析MTHFR C677T基因型与侧支循环等临床资料的关系。 结果MTHFR C677T基因多态性与脑梗死CISS分型差异无显著性,观察组MTHFR C677T基因TT型患者血清Hcy高于CT型和CC型患者(P<0.05)。侧支循环良好、颈动脉中重度狭窄及治疗后有复发患者MTHFR C677T基因TT基因型比例明显高于侧支循环不良、颈动脉非中重度狭窄及治疗后无复发患者(P<0.05);颈动脉非中重度狭窄、治疗后无复发患者MTHFR C677T基因TC基因型比例高于颈动脉中重度狭窄和治疗后有复发患者(P<0.05);侧支循环不良、治疗后无复发患者MTHFR C677T基因CC基因型比例高于侧支循环良好和治疗后有复发患者(P<0.05)。 结论MTHFR C677T基因多态性与脑梗死CISS分型无明显关系,但与患者血清Hcy、侧支循环、颈动脉中重度狭窄以及治疗后复发有关。 |

AimTo explore the relationship between methylenetetrahydrofolate reductase (MTHFR) C677T gene polymorphism and China ischemic stroke subclassification (CISS) and cerebral collateral circulation in cerebral infarction. MethodsA total of 167 inpatients with ischemic cerebral infarction were selected as the observation group, and 160 healthy volunteers were selected as the control group. The difference in the distribution of MTHFR C677T genotype between the two groups was examined, and the distribution of MTHFR C677T genotype in patients with different CISS subtypes was also analyzed, as well as the difference in serum homocysteine (Hcy) in patients with different MTHFR C677T genotype distributions, and the relationship between the MTHFR C677T genotype and clinical data such as collateral circulation. Results There is no significant difference between the MTHFR C677T gene polymorphism and the CISS typing of cerebral infarction, the proportion of MTHFR C677T gene TT genotype in patients with good collateral circulation, moderate to severe carotid artery stenosis and recurrence after treatment was significantly higher than that in patients with poor collateral circulation, non-moderate to severe carotid artery stenosis and no recurrence after treatment (P<0.05). The proportion of TC genotype of MTHFR C677T gene in patients with non-moderate to severe carotid artery stenosis and no recurrence after treatment was higher than that in patients with moderate to severe carotid artery stenosis and patients with recurrence after treatment (P<0.05). The proportion of MTHFR C677T gene CC genotype in patients with poor collateral circulation and no recurrence after treatment was higher than that in patients with good collateral circulation and patients with recurrence after treatment (P<0.05). ConclusionMTHFR C677T gene polymorphism has no obvious relationship with CISS classification of cerebral infarction, but it is related to serum Hcy, collateral circulation, moderate and severe stenosis of carotid artery and recurrence after treatment. |
