目的探讨脐动脉血流动力学和血乳酸与宫内窘迫胎儿缺氧程度、氧化应激程度的关系。 方法选取122例孕妇,其中60例为宫内窘迫组(Apgar评分<7分),62例为对照组(Apgar评分≥7分)。比较两组脐动脉血流动力学指标、新生儿血乳酸、氧化应激指标;评价脐动脉血流动力学和血乳酸与缺氧程度、氧化应激程度的关系。 结果宫内窘迫组脐动脉阻力指数(RI)、脐动脉搏动指数(PI)、收缩期和舒张期流速比(S/D)、乳酸、8-羟基脱氧鸟苷(8-OHDG)、丙二醛(MDA)高于对照组;谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)、过氧化氢酶(CAT)低于对照组(P<0.05)。RI、PI、S/D和乳酸均与8-OHDG、MDA呈负相关,均与GSH-Px、SOD、CAT、窒息程度呈正相关(P<0.05)。 结论胎儿发生宫内窘迫时,脐动脉血流动力学、血乳酸与胎儿缺氧程度、氧化应激程度存在相关性,可为临床诊断胎儿宫内窘迫提供参考。 |

AimTo investigate the relationship between umbilical artery hemodynamics, blood lactic acid and the degree of fetal hypoxia and oxidative stress in intrauterine distress. MethodsA total of 122 pregnant women were selected, of which 60 were in the intrauterine distress group (Apgar score<7) and 62 were in the control group (Apgar score ≥7). The indexes of umbilical artery hemodynamics, neonatal blood lactic acid and oxidative stress were compared between the two groups; To evaluate the relationship between umbilical artery hemodynamics and blood lactic acid and the degree of hypoxia and oxidative stress. ResultsThe umbilical artery blood RI, PI, S/D, lactic acid, 8-OHDG, MDA in the intrauterine distress group were higher than those in the control group; GSH Px, SOD and CAT were lower than those in the control group (P<0.05). RI, PI, S/D and lactic acid were negatively correlated with 8-OHDG and MDA, and positively correlated with GSH Px, SOD, CAT and asphyxia degree (P<0.05). ConclusionWhen fetal distress occurs, umbilical artery hemodynamics and blood lactic acid are related to the degree of fetal hypoxia and oxidative stress, which can provide reference for clinical diagnosis of fetal distress. |
