目的分析γ-干扰素(IFN-γ)、Toll样受体4(TLR4)、转化生长因子-β1(TGF-β1)对呼吸道合胞病毒(RSV)感染毛细支气管炎患儿病情严重程度的预测价值。 方法选取82例RSV感染毛细支气管炎患儿,依据病情严重程度分为轻度组、中度组、重度组。多因素Logistic回归分析RSV感染患儿IFN-γ、TLR4、TGF-β1水平以及病情严重程度影响因素。Spearman分析IFN-γ、TLR4、TGF-β1与病情严重程度的相关性。绘制受试者工作特征(ROC)曲线分析IFN-γ、TLR4、TGF-β1水平预测RSV感染毛细支气管炎病情的价值。 结果中、重度组的年龄、IFN-γ水平均低于轻度组且重度组更低,中度组、重度组的早产儿比例、TLR4、TGF-β1水平高于轻度组且重度组更高(P<0.05);RSV感染患儿病情严重程度的保护因素是年龄、IFN-γ,危险因素是TLR4、TGF-β1、早产儿;病情严重程度与TLR4、TGF-β1呈正相关,与IFN-γ水平呈负相关;IFN-γ、TLR4、TGF-β1预测RSV感染毛细支气管炎病情均AUC>0.70,灵敏度>77%,特异度>60%。 结论TLR4、TGF-β1、IFN-γ对RSV感染毛细支气管炎患儿病情严重程度具有较好的预测价值。 |

AimAnalyze interferon-γ(IFN-γ), Toll-like receptor 4(TLR4), transforming growth factor-β1(TGF-β1) to predict the severity of respiratory syncytial virus (RSV) infection in children with bronchiolitis. MethodsEighty-two cases of RSV-infected children with bronchiolitis were selected and divided into mild group, moderate group and severe group according to the severity of the disease. The levels of IFN-γ, TLR4 and TGF-β1 in RSV infected children and the influencing factors of the severity of the disease in clinical data were analyzed by multivariate Logistic regression. The correlation between IFN-γ, TLR4 and TGF-β1 and the severity of the disease was analyzed by Spearman The value of IFN-γ, TLR4 and TGF-β1 in predicting the disease of RSV-infected bronchiolitis was analyzed by receiver operating characteristic (ROC) curve. ResultsThe age and IFN-γ levels of the moderate and severe groups decreased than those of the mild group, and the index levels of the severe group decreased. The proportion of premature infants, TLR4 and TGF-β1 levels of the moderate and severe groups increased than those of the mild group, and all the values of the severe group increased(P<0.05); the protective factors for the severity of RSV infection were age and IFN-γ, while the risk factors were TLR4, TGF-β1 and premature infant; the severity of the disease was positively correlated with TLR4 and TGF-β1, and negatively correlated with IFN-γ levels; the area under the ROC curve of IFN-γ, TLR4, and TGF-β1 in the evaluation of RSV-infected bronchiolitis in children with ROC curve line AUC are all above 0.70, the sensitivity is above 77%, and the specificity is above 60%. ConclusionTLR4, TGF-β1 and IFN-γ has a good predictive value for the severity of RSV infection in children with bronchiolitis. |
