目的探讨程序化间歇硬膜外麻醉(PIEB)对妊娠期高血压疾病(HDP)剖宫产麻醉效果和对血流动力学的影响。 方法选择行剖宫产HDP患者98例,随机分为连续组(连续硬膜外麻醉)和间歇组(PIEB)。比较两组麻醉效果、血流动力学变化、不良反应及新生儿情况。 结果间歇组感觉和运动阻滞起效时间、达最高平面时间短于连续组,持续时间长于连续组(P<0.05)。间歇组T1、T2时MAP高于连续组,T1、T2、T4时HR高于连续组(P<0.05)。间歇组不良反应发生率低于连续组(P<0.05)。两组新生儿情况比较差异无显著性(P>0.05)。 结论程序化间歇硬膜外麻醉HDP剖宫产的麻醉效果佳,可有效维持术中血流动力学稳定,且安全性较高。 |

AimTo explore the effect of programmed intermittent epidural boluses (PIEB) on the anesthesia effect and hemodynamics of hypertensive disorders of pregnancy (HDP) cesarean. Methods98 HDP patients with HDP undergoing cesarean section were randomly divided into continuous group (continuous epidural anesthesia) and intermittent group (PIEB). The anesthetic effects, hemodynamic changes, adverse effects, and neonatal conditions were compared between the two groups. ResultsThe onset time of sensory and motor block and the time of reaching the highest plane in the intermittent group were shorter than those in the continuous group, and the duration was longer than those in the continuous group (P<0.05). MAP was higher in the intermittent group at T1 and T2 than that in the continuous group, and HR at T1, T2 and T4 was higher than that in the continuous group (P<0.05). The incidence of adverse effects in the intermittent group was lower than that in the continuous group (P<0.05). There was no significant difference between the two neonatal groups (P>0.05). ConclusionProgrammed intermittent epidural anesthesia HDP cesarean section has effective intraoperative hemodynamic stability and high safety. |
