| 目的研究未足月胎膜早破合并宫内感染产妇胎盘NOD样受体蛋白3(NLRP3)、白细胞介素-1β(IL-1β)、Fasl的表达及其临床意义。 方法选择未足月胎膜早破产妇118例,根据是否合并宫内感染分为感染组36例和无感染组82例。比较两组胎盘NLRP3阳性检出率、IL-1β、Fasl表达及血清β-绒毛膜促性腺激素(β-HCG)、降钙素原(PCT)、C反应蛋白(CRP)水平,并分析其诊断合并宫内感染的临床价值。 结果感染组胎盘NLRP3阳性检出率、IL-1β、Fasl mRNA、血清β-HCG、PCT、CRP水平高于无感染组(P＜0.05)。NLRP3、IL-1β、Fasl与β-HCG、PCT、CRP均呈正相关(P＜0.05)。NLRP3、IL-1β、Fasl诊断未足月胎膜早破合并宫内感染的效能均较高,其中Fasl效能最高。 结论未足月胎膜早破合并宫内感染产妇胎盘NLRP3阳性检出率、IL-1β、Fasl表达均较高,此3指标对诊断胎膜早破合并宫内感染有较高的临床价值。 |

| AimTo study the expressionsand clinical significance of NOD-like receptor protein 3 (NLRP3), interleukin 1β (IL-1β) and Fasl in the placenta of puerperae with preterm premature rupture of membranes complicated with intrauterine infection. Methods118 patients with preterm premature rupture of membranes were selected and divided into infected group (36 cases) and non-infected group (82 cases) according to whether they were complicated with intrauterine infection. The expressions of NLRP3, IL-1β and Fasl in the placenta and levels of serum β-chorionic gonadotropin (β-HCG), procalcitonin(PCT) and C-reactive protein (CRP) were compared between the two groups, and the clinical value in the diagnosis ofintrauterineinfection was analyzed. ResultsThe positive detection rate of NLRP3 in the placenta and levels of IL-1β, Fasl mRNA, serum β-HCG, PCT and CRP in infected groupwerehigher thannon-infected group(P＜0.05). NLRP3, IL-1β and Fasl were positively correlated with β-HCG, PCT and CRP (P＜0.05). The NLRP3, IL-1β and Fasl had high efficiencies in diagnosing preterm premature rupture of membranes with intrauterineinfection, and the efficacy of FasL is the highest. ConclusionThe expressions of NLRP3, IL-1β and Fasl in the placenta are higher among patients with preterm premature rupture of membranes complicated with intrauterine infection. These three indicators havehigh clinical value in the diagnosis of premature rupture of membranes with intrauterine infection. |
