目的探讨阴道微环境和人乳头瘤病毒(HPV)感染与宫颈上皮内瘤变(CIN)的相关性。 方法选择宫颈低级别上皮内瘤变(LSIL)患者72例(LSIL组)、高级别上皮内瘤变(HSIL)患者45例(HSIL组)和正常宫颈者104例(对照组)。比较各组临床资料,行多因素Logistic回归分析阴道微环境和HPV感染与宫颈上皮内瘤变的相关性。 结果Logistic回归分析发现,HPV感染、细菌性阴道病(BV)、外阴阴道假丝酵母菌病(VVC)、白细胞介素-2(IL-2)、IL-4、干扰素-γ均为LSIL发生的独立危险因素。首次性生活年龄、HPV感染、阴道pH、BV、滴虫性阴道炎、VVC、IL-2、IL-4、IFN-γ均为HSIL发生的独立危险因素。 结论阴道微环境和HPV感染均可能增加CIN发生,两者均为影响CIN发生发展的重要影响因素。 |

To investigate the relationship between vaginal micro-environment, human papillomavirus (HPV) infection and cervical intraepithelial neoplasia (CIN). Methods72 patients in low-grade squamous intraepithelial lesion(LSIL) group, 45 patients in high-grade squamous intraepithelial lesion (HSIL) group and 104 healthy controls in control group were enrolled. The clinical data were compared in different groups. Correlation between vaginal micro-environment, HPV infection and CIN was analyzed by multivariate Logistic regression analysis. ResultsLogistic regression analysis showed that HPV infection, bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), interleukin-2 (IL-2), IL-4 and interferon-γ (IFN-γ) were independent risk factors of LSIL. The age of the first sexual life, HPV infection, vaginal pH, BV, trichomonas vaginitis (TV), VVC, IL-2, IL-4 and IFN-γ were all independent risk factors of HSIL. ConclusionVaginal micro-environment and HPV infection may increase the incidence of CIN, and they are important influencing factors for the occurrence and development of CIN. |
