      目的分析ATA、ACR TI-RADS和C-TIRADS分类标准在甲状腺结节鉴别诊断中的应用价值。 方法选取甲状腺结节患者238例(265个结节),常规超声检查并进行统计学分析。按照ATA、ACR TI-RADS和C-TIRADS标准对结节进行分类诊断,以病理结果为金标准绘制ROC曲线预测3种方法的诊断价值。 结果265个甲状腺结节中良性结节128个(48.30%),恶性结节137个(51.70%)。ATA、ACR TI-RADS、C-TIRADS灵敏度分别为97.81%、94.19%、91.24%,曲线下面积分别为0.807、0.815、0.905。C-TIRADS的诊断效能高于ATA、ACR TI-RADS分类标准(P<0.05)。 结论三种分类方法对甲状腺结节均有一定的诊断价值,C-TIRADS对甲状腺结节分类诊断价值最高。

      To analyze the application value of ATA guidelines, ACR TI-RADS classification and C-TIRADS classification in the differential diagnosis of thyroid nodules. Methods238 patients with thyroid nodules (265 nodules) were selected for routine ultrasound examination and statistical analysis. The nodules were classified and diagnosed according to ATA, ACR TI-RADS and C-TIRADS and the pathological results were used as the gold standard to draw ROC curve to predict the diagnostic value of the three methods. ResultsAmong 265 thyroid nodules, benign nodules were 128(48.30%) and malignant nodules were 137(51.70%). The sensitivity of ATA, ACR TI-RADS and C-TIRADS is 97.81%, 94.19% and 91.24% respectively, and the area under the curve is 0.807,0.815 and 0.905 respectively. The diagnostic efficiency of C-TIRADS is higher than that of ATA and ACR TI-RADS classification standards. ConclusionThe three classification methods have certain diagnostic value for thyroid nodules, and C-TIRADS has a broad application prospect in clinical practice.
