目的分析血清半乳糖凝集素-3(Gal-3)、同型半胱氨酸(Hcy)与急性心肌梗死(AMI)患者心力衰竭(HF)程度及预后的相关性。 方法将80例AMI患者根据是否合并HF分为HF组35例和单纯AMI组45例。根据Killip分级判断患者HF程度,ELISA检测血清Gal-3、Hcy水平。HF患者根据预后情况分为预后良好组19例和预后不良组16例。Logistic回归分析影响AMI预后不良的危险因素,ROC曲线分析Gal-3、Hcy水平预测AMI合并HF患者的预后价值。 结果HF组Gal-3、Hcy水平高于单纯AMI组(P<0.05)。HF患者Ⅳ级Gal-3、Hcy水平高于Ⅲ级和Ⅱ级,Ⅲ级Gal-3、Hcy水平高于Ⅱ级(P<0.05)。预后不良组年龄、糖尿病史、高脂血症史、心界扩大、服用抗凝药、Gal-3、Hcy水平与预后良好组比较差异有显著性(P<0.05)。多因素分析显示,年龄、Gal-3、Hcy是AMI合并HF患者预后的影响因素(P<0.05)。Gal-3和Hcy联合预测AMI患者HF后预后情况的灵敏度、AUC高于两指标单一检测(P<0.05)。 结论血清Gal-3、Hcy水平随AMI患者HF级别递增而升高,两者联合预测AMI患者的诊断效能最高。 |

To analyze the correlation between serum galectin-3(Gal-3) and homocysteine (Hcy) and the severity and prognosis of heart failure (HF) in patients with acute myocardial infarction (AMI). Methods80 patients with AMI were divided into HF group (n=35) and simple AMI group (n=45) according to whether they were complicated with HF. The serum levels of Gal-3 and Hcy were detected by ELISA. According to the prognosis, HF patients were divided into good prognosis group (19 cases) and poor prognosis group (16 cases). Logistic regression was used to analyze the risk factors of poor prognosis of AMI. ROC curve was used to analyze the prognostic value of Gal-3 and Hcy levels in predicting AMI patients with HF. ResultsThe levels of Gal-3 and Hcy in HF group were higher than those in AMI group (P<0.05). The levels of Gal-3 and Hcy in grade Ⅳ were higher than those in grade Ⅲ and Ⅱ, and the levels of Gal-3 and Hcy in grade Ⅲ were higher than those in grade Ⅱ (P<0.05). Age, history of diabetes and hyperlipidemia, heart boundary expansion, anticoagulants, Gal-3, Hcy levels in poor prognosis group were sigificantly different from those in the group with good prognosis(P<0.05). Multivariate analysis showed that age, Gal-3 and Hcy were the prognostic factors of AMI patients with HF (P<0.05). The sensitivity and AUC of Gal-3 and Hcy combined in predicting the prognosis of AMI patients after HF were higher than those of the two indexes alone (P<0.05). ConclusionThe expression of serum Gal-3 and Hcy levels increased with the increase of HF level in AMI patients, and the combination of them had the highest diagnostic efficiency in predicting AMI patients. |
