      目的探讨重症腺病毒肺炎(SAP)患儿血清巨噬细胞炎症蛋白-1α(MIP-1α)、转化生长因子-β1(TGF-β1)、白细胞介素-10(IL-10)水平及其临床意义。 方法选择腺病毒肺炎(AP)患儿118例,分为SAP组57例和非SAP组61例,将SAP患儿根据预后分为有效组和无效组。比较各组治疗前、治疗4天、治疗7天血清MIP-1α、TGF-β1、IL-10水平,及其对SAP诊断和预后的效能。结果与治疗前比较,MIP-1α、TGF-β1、IL-10水平治疗后两组均随时间逐步下降,且治疗前、治疗4天、治疗7天SAP组均高于非SAP组(P<0.05)。治疗后有效组MIP-1α、TGF-β1、IL-10水平低于无效组(P<0.05)。MIP-1α、TGF-β1、IL-10三者联合检测对SAP诊断和预后预测的效能最高(P<0.05)。结论SAP患儿血清MIP-1α、TGF-β1、IL-10水平高,三者联合检测对SAP诊断和预后预测有重要价值。

      To investigate the serum levelsand clinical significance of serum macrophage inflammatory protein-1α (MIP-1α),transforming growth factor-β1 (TGF-β1) and interleukin-10 (IL-10) in children with severe adenovirus pneumonia (SAP). Methods118 children with adenovirus pneumonia (AP) were selected and divided into SAP group (n=57) and non-SAP group (n=61). Patients with SAP were divided into effective group and ineffective group according to their prognosis. The serum levels of MIP-1α, TGF-β1 and IL-10 in each group were compared before treatment, 4 days after treatment and 7 days after treatment, and their efficacy on the diagnosis and prognosis of SAP were compared. ResultsCompared with before treatment, the levels of MIP-1α, TGF-β1 and IL-10 in the two groups decreased gradually with time after treatment, and their levels in the SAP group were higher than those in the non-SAP group before treatment, 4 days and 7 days after treatment (P<0.05). After treatment, the levels of MIP-1α, TGF-β1 and IL-10 in the effective group were lower than those in the ineffective group (P<0.05). The combined detection of MIP-1α, TGF-β1 and IL-10 had the highest efficacy in the diagnosis and prognosis of SAP (P<0.05). ConclusionMIP-1α,TGF-β1 and IL-10 are in high level in the serum of children with SAP, and the combined detection of MIP-1α, TGF-β1 and IL-10 is of great value for the diagnosis and prognosis of SAP.
