| 目的探讨真空辅助阴道分娩(VAVD)牵引力与新生儿不良结局的关系。 方法选择初产妇351例,按照牵引力分为高牵引组和正常牵引组。比较两组临床特征和新生儿结局,分析新生儿不良结局的危险因素。 结果孕周、胎头位置中、牵引次数、杯分离、新生儿体质量、牵引失败等情况两组差异有显著性(P＜0.05)。高牵引组入新生儿重症监护病房(NICU)、5 min Apgar评分＜7分、新生儿缺氧缺血性脑病(HIE)等新生儿不良结局占比均高于正常牵引组(P＜0.05)；高牵引力是新生儿入NICU和HIE的危险因素(P＜0.05)。 结论VAVD高牵引力与新生儿不良结局相关,高牵引力是新生儿入NICU和HIE的危险因素。 |

| To study the relationship between the traction of vacuum assisted vaginal delivery (VAVD) and the adverse outcome of newborns. Methods351 Primiparas were selected, and divided into high traction group and normal traction group according to traction force. The clinical characteristics and neonatal outcomes were compared between this two groups, and the risk factors of adverse neonatal outcomes were analyzed. ResultsThere were significant differences in gestational weeks, fetal head position, traction times, cup separation, neonatal body mass, traction failure between the two groups (P＜0.05). The proportion of neonatal adverse outcomes such as NICU, 5-min Apgar score ＜7 and HIE in high traction group were higher than those in normal traction group (P＜0.05); High traction was a risk factor for neonatal admission to NICU and HIE (P＜0.05). ConclusionHigh traction force in VAVD is associated with poorneonataloutcomes. High traction forceis a risk factor for neonatal admission to NICU and HIE. |
