目的探讨胸痛患者血清高敏肌钙蛋白(hs-cTnT)、微小RNA-448(miR-448)水平对急性冠脉综合征(ACS)的诊断价值。方法选择因胸痛入院的ACS患者206例为ACS组,根据Gensini评分将其分为轻、中、重度狭窄组;另择同期因胸痛入院的非ACS患者100例为对照组。比较各组临床资料和血清hs-cTnT、miR-448水平,分析ACS危险因素和miR-448与Gensini评分等指标的相关性,以及miR-448、hs-cTnT对ACS的诊断价值。结果ACS组hs-cTnT、miR-448较对照组升高(P<0.05);轻、中、重度狭窄组miR-448水平依次升高(P<0.05)。miR-448、hs-cTnT、心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)、低密度脂蛋白胆固醇(LDLC)是ACS危险因素。ACS患者miR-448水平与hs-cTnT、cTnI、CK-MB、甘油三酯、总胆固醇、LDLC、Gensini评分呈正相关(P<0.05),与高密度脂蛋白胆固醇呈负相关(P<0.05)。miR-448、hs-cTnT联合cTnI和CK-MB诊断ACS效能更高。结论血清miR-448、hs-cTnT对ACS早期诊断有一定价值,联合诊断效能更高。 |

To investigate the diagnostic value of serum high-sensitivity cardiac troponin T (hs-cTnT) and microRNA-448 (miR-448) levels in patients with chest pain in acute coronary syndrome (ACS). MethodsA total of 206 ACS patients admitted to hospital due to chest pain were selected as the ACS group and divided into mild, moderate and severe stenosis groups according to the Gensini score; 100 non-ACS patients admitted to the hospital due to chest pain during the same period were selected as the control group. The clinical data and serum hs-cTnT and miR-448 levels in each group were compared, and the correlation of ACS risk factors and miR-448 with Gensini score, as well as the diagnostic value of miR-448 and hs-cTnT in ACS were analyzed. ResultsThe levels of hs-cTnT and miR-448 in the ACS group were higher than those in the control group(P<0.05); the levels of miR-448 in the mild, moderate and severe stenosis groups were increased in turn(P<0.05). miR-448, hs-cTnT, cardiac troponin I (cTnI), creatine kinase isoenzyme (CK-MB), and low-density lipoprotein cholesterol (LDLC) were risk factors for ACS. The level of miR-448 in ACS patients was positively correlated with hs-cTnT, cTnI, CK-MB, triglyceride, total cholesterol, LDLC, and Gensini score(P<0.05), and was negatively correlated with high-density lipoprotein cholesterol(P<0.05). The combination of miR-448, hs-cTnT, cTnI and CK-MB was more effective in diagnosing ACS. ConclusionSerum miR-448 and hs-cTnT have certain value in the early diagnosis of ACS, and the combined diagnosis of cTnI and CK-MB has a higher diagnostic efficiency. |
