      目的调查玉林市2017—2019年新生儿呼吸道感染的病原菌分布和耐药情况。方法以2017—2019年玉林市7家医院收治的1 594例呼吸道感染新生儿为研究对象,收集其痰液样本进行菌株分布及耐药性分析。结果共分离出病原菌菌株1 400株(87.83%),排名前五位依次为肺炎克雷伯菌、大肠埃希菌、金黄色葡萄球菌、鲍曼不动杆菌、铜绿假单胞菌。检出多重耐药菌688株,排名前三位为产超广谱β-内酰胺酶(ESBLS)肺炎克雷伯菌、耐碳青霉烯类鲍曼不动杆菌、产-ESBLS大肠埃希菌。产-ESBLS肺炎克雷伯菌对妥布霉素、左氧氟沙星敏感。耐碳青霉烯类鲍曼不动杆菌对替加环素及阿卡米星高度敏感。结论新生儿呼吸道感染的病原菌以革兰氏阴性菌为主,多重耐药菌以产-ESBLS为主。

      To investigate the distribution and drug resistance of pathogenic bacteria in neonatal respiratory tract bacterial infections in Yulin from 2017 to 2019. MethodsA total of 1 594 neonates with respiratory tract infection from 7 hospitals in Yulin from 2019 to 2021 were selected as the research object, which sputum samples were collected and monitored for bacterial distribution and drug resistance. ResultsA total of 1 400 strains of pathogenic bacteria (87.83%) were isolated, the top five pathogens were Klebsiella pneumoniae, Escherichia coli, Acinetobacter baumannii, Staphylococcus aureus, and Pseudomonas aeruginosa. 688 strains of multidrug-resistant bacteria were detected, and the top three were extended-spectrum β-lactamase (ESBLS)-producing Klebsiella pneumoniae, carbapenem-resistant Acinetobacter baumannii, and ESBLS-producing Escherichia coli. ESBLS-producing Klebsiella pneumoniae is sensitive to tobramycin, and levofloxacin. Carbapene-resistant Acinetobacter baumannii is sensitive to tigecycline and akamicin. ConclusionThe pathogenic bacteria of neonatal respiratory tract infection are mainly gram-negative bacteria and the multi-drug resistant bacteria were mainly ESBLS-producing.
